things to look for in an assisted living facility or skilled nursing home BY MARIE PETERSON Long Term Care Ombudsman A re you sick and tired of being sick and tired? Are you bored and want to have fun with a group of active seniors and not worry about cook- ing, cleaning or yard work? Then an assisted living center might be for you. Have you just had surgery or a stroke and have been in the hospital? Do you need wound care or physical therapy to make you stronger? Then a skilled nursing home might be for you. And, Medicare might pay for it if you have been in the hospital three nights, the doctor writes an order for skilled nursing based on your hospital diagnosis and you go to the skilled nursing within 30 days of your hospital discharge. Do you or your loved one need minimal or moderate care, medica- tion management or supervision because of memory loss? Is it unsafe for you or your loved one to remain at home? Then either an assisted living center or a skilled nursing home might be for you. There is a difference between assisted living centers and skilled nursing homes. Be sure your choice meets your needs. Ask questions. This set of questions or check- list will help you know what to look for when trying to decide whether an assisted living center or a skilled nursing and rehabilita- tion facility (nursing home) might be the best choice for you or your loved one. These are general ques- tions to ask about both types of facilities. If you or your loved one has special needs, be sure to ask if the facility can accommodate them. Before making your decision: Always visit, always test a meal or two. Always check activities. Always feel comfortable. Here are 10 things to check for when researching a facility: Geography: Is the facility located conveniently for visitors and preferred hospitals? Financial: What is the daily/monthly cost? What does this fee include? What are the addition- al fees? Is the facility receptive to accepting Medicare, Medicaid, VA benefits, insurance, private pay? Diagnosis: Is the facility able to provide the appropriate level of care? Is the staff trained in the diagnosis of your loved one? How do they monitor wandering resi- dents? Personal care and staff atti- tude: Is the staff polite and courte- ous to the residents and to you? Is there enough staff? Are call lights answered promptly? Are the resi- dents clean, neat and tidy? Is per- sonal hygiene addressed adequate- ly? Are name tags visible? Resident attitude: Do the majority of the residents appear comfortable and content? Are they smiling? Activities: Are there group activities? Are there individual activities? Are there weekend activities? Are there outings? Is exercise equipment available? Activities make life worth living. Choice: What choices are available in activities, room selec- tion, menu, etc.? Are you able to keep a pet? Is there a hair dresser? Is there a suggestion box? Housekeeping: Is the facility neat and clean? Are spills cleaned up quickly? Dietary: Does the monthly menu show adequate variety? Will they accommodate your particular diet and food preferences? Did you try a meal? Was it tasty and pre- sented well? General atmosphere and appearance: Is the facility inviting, comfortable and home-like? Is the outside area easily accessible? Is there adequate protection from sun or rain? Ask to see the latest survey report from the Bureau of Facility Standards. The survey will point out problems noted during their last visit. The latest surveys are located in the facility or at www.facilitystandards.idaho.gov. Another resource, www.medicare.gov, compares nursing homes throughout the country. Do you prefer a shower or a bath? Would you prefer your shower/bath in the morning, afternoon or evening? What are your favorite foods? What foods don’t you like? Do you have any allergies? What do you like to do? Read? Listen to music? Pet your dog? Garden? Watch a movie? Just leave me alone. What help do you need? Do you have favorite clothes that you like to wear? What time do you like to go to bed at night? You have the right to make choices about your care. You are the consumer. Quality care includes all aspects of living, not just physical needs. Have a care plan meeting. Doing the things you like can make your day better. How can you get your needs known by the staff? At a care plan meeting, also known as a care conference, all your needs can be discussed and decisions can be made to include your choices. Your decisions are written down in cate- gories so that all staff will under- stand what care you need, what you like and dislike and what you like to do. This information is shared with the staff so that they know who you are and what you need to make your stay worthwhile. Know who to go to if you have a problem. No matter how well a plan is made, sometimes a problem may develop. Knowing who to go to when this happens is critical. Get a list of the staff that is responsible for day to day care (administrator, director of nursing, charge nurse or house man- ager). Each facility has its own staff organization. If the facility is not addressing your concerns to your sat- isfaction, you have the right to call your long term care ombudsman, whose job it is to investigate com- plaints and work with the resident and staff toward resolution. Another avenue to follow is the state regulato- ry agency, Health and Welfare’s Bureau of Facility Standards and Licensing and Certification. You can reach the bureau at (208) 334-6626. They are also responsible to answer questions about rules and regulations and investigate concerns or prob- lems. Whether you or someone youlove needs care at either a nursinghome or an assisted living center, itshould be the best it can be. Howdo you get good quality care?

Communication is important to leteveryone know what is needed. Howwould you like to start your day?When would you like to get up? Doyou like to sleep in? Do you prefer ashower or a bath? Would you preferyour shower/bath in the morning,afternoon or evening? What are yourfavorite foods? What foods don’t youlike? Do you have any allergies?What do you like to do? Read? Listento music? Pet your dog? Garden?Watch a movie? Just leave me alone.What help do you need? Do you havefavorite clothes that you like to wear?What time do you like to go to bed atnight? You have the right to makechoices about your care. You are theconsumer. Quality care includes all

aspects of living, not just physicalneeds.

Have a care plan meeting. Doingthe things you like can make yourday better. How can you get yourneeds known by the staff? At a careplan meeting, also known as a careconference, all your needs can bediscussed and decisions can bemade to include your choices. Yourdecisions are written down in cate-gories so that all staff will under-stand what care you need, what youlike and dislike and what you like todo. Persevering the challenges of raising my grandchild Across America more

than six millionchildren are living

in grandparent (or otherrelative caregiver) house-holds. The main caregiverof many of these childrenis a grandparent, and I am

one of these grandpar-ents.

My introduction to theGrandparents RaisingGrandchildren Programand Emily came a fewmonths after my grand-daughter was born. The

knowledge and insightEmily has plus the sup-port I’ve received fromother grandparents in theprogram has helped pullme through this difficultyear.

My daughter suffers

from schiz-ophreniaand strug-gles dailywith her ill-ness as wellas sideeffects ofher medica-tion. While

she was pregnant I imag-ined that she would needa lot of help with her newaddition. I hadn’t realizedwhat an overwhelmingtask I had before me.After the birth my daugh-ter could not wake up tocare for or feed her newbaby. As her health start-ed to improve she spentmore time with her baby.The burden of care stillfell on me for nighttimefeedings and most of thedaily care. I was the oneto see that my grand-daughter’s needs wereconsistently met.Numerous hospitaliza-tions complicated mydaughter’s ability to par-ent her daughter. Thiscontinues today, morethan a year later.

It was after the secondhospitalization that Isought out theGrandparents RaisingGrandchildren Program.Emily listened to my storyand shared some of hervast health of informationwith me. I could workwith legal aid for theguardianship. TheDepartment of Health andWelfare could help mekeep my granddaughteron Medicaid. The annualback to school picnic wascoming up, so I couldmeet and socialize withother grandparents. Ifound out that she was

looking for a new employ-ee in her office. I appliedfor the job. A couplemonths later I was hired.

Working with theGrandparents RaisingGrandchildren Programhas been wonderful forme. I have met manywonderful people, andfeel the support of othergrandparents, the adviso-ry council as well as thesupport of the community.

I have to admit, thatalthough I adore mygrandbaby, I was ill pre-pared in many ways. Thefinancial burden has beenhardest to bear. Mostgrandparents qualify fora “child only” TAFI grantthrough the departmentof Health and Welfare for$309. But because I carefor my daughter as wellas my granddaughter I donot qualify for this grant.I was able to qualify forICCP, but the co-pay por-tion of my bill, whichincludes my granddaugh-ter, my 5 year old andtwo school-age childrenwas more than my netpay per month. I felttrapped!

I have overcome theproblem of childcarecosts, but I’m sure this isonly the first of many dif-ficult situations I will facein the future. Knowingthat I am not alone andthat there are avenues toturn to for support hasmade me feel more pre-pared for what lies ahead.I will survive, in fact I willpersevere and as a familywe will accomplish greatthings!

Fraud Alert: This scam has beentraveling across the United States

Individuals in the states of New York, Maryland and California arereporting receiving calls from a person claiming to be from a medicaloffice wanting to send the individual a new Medicare card.

They already had the person’s name, address, social security num-ber, and they just needed to confirm the bank account number so theycould take the $48 fee for the card out of their account.

Unfortunately, most of the recipients of the call gave out theirbank information to the caller. Later they found out the call wasbogus and notified their bank and the police.

Remember, never give out your bank information to someonewho has called you.

The Silver Sneakersprogram for seniorcitizens has a simple

goal: “Get fit, have fun,make friends.”

Apple Athletic is one ofabout 10,000 sites nation-wide that offers classesthrough Silver Sneakersto senior citizens, 65 andolder.

The program is offeredthrough a HumanaMedicare supplementunder MedicareAdvantage and supplies afree membership to clubssuch as Apple AthleticClub.

“We have lots of class-es with music that’s ageappropriate for 50-year-olds to people in their90s,” Laurie Wright, aninstructor at AppleAthletic said. “The pur-pose is to strengthen themand give them skills foractive, daily living.”

Incorporated into theSilver Sneakers classes isimproving increasedstrength, balance, coordi-nation, flexibility, range of

motion and fall preven-tion.

“It’s all of those thingsthat make the things theywant to do in their dailylives more enjoyable,”Wright said. “It keepsthem strong.”

At 70 years old, PattyCushman, of Idaho Falls,works out at AppleAthletic five days a week.

“Silver Sneakers savedme so much money on

Humana,” Cushman said.“I was paying $600 a yearbefore this.”

Cushman enjoys theyoga and water aerobicsclasses and the fact thatthe exercise has improvedher health.

“Every senior citizenshould do it,” Cushmansaid. “It helps preventfalls.”

Sandi Gordon, a trainerat Apple Athletic, said

Silver Sneakers club mem-bers have unrestrictedaccess to the club, just asany other member would.

Wright said the optionsavailable for seniors alsoinclude social and educa-tional benefits.

“It really is a social net-work for a lot of them,”Wright said. “Especiallythose that have lost aspouse or they’re single.It’s a very social thing thatgets them up and out ofthe house to fight depres-sion. It keeps them active,keeps them moving.”

The activities hosted byApple Athletic help formfriendships.

“We also have socials,educational workshopsthat are age appropriate,and they become friends,”Wright said. “It’s a socialthing for their better well-being. As a senior, it helpswith depression.”

Recent educationalworkshops the club host-ed included cooking forone or two people. Theclub also recently had aworkshop on osteoporo-sis.

“You fulfill the socialoutlet which makes themjust feel better aboutthemselves,” Wright said.“They feel more alive,more vibrant, but the ben-efit of the exercise is justgreat. It’s a great supportsystem and I think that’sworth as much as theexercise benefit.”

Gordon said eachcourse could be adjustedto suit whatever a senior’sneeds are. She has hadpeople come in whorecently had heart andkidney transplants, sever-al who have had cancer,multiple sclerosis, kneereplacement and hipreplacements.

“I used to have severalthat came to class withoxygen and their canes,”Wright said. “Some even-tually don’t need to usetheir cane anymore andthey’re so excited becauseit’s a big deal, it really is.It gives them more inde-pendence.”

Providing servicesfor seniors: Todayand tomorrow

Earlier this month, I had the opportunity to presentat the monthly Geriatric Forum held at EIRMC. Inall honesty, until then, the magnitude of what is

offered for seniors through the Area Agency on Aginghad not hit me in my two years as the Director of the

Area Agency on Aging. During my day, my focus shifts from

one program to another as my staff and Iwork diligently for the seniors of easternIdaho.

Before I go on, I want to thank thestaff of the Area Agency on Aging,because they are the lifeblood of what wedo!

There are a few staples of what anArea Agency on Aging does, including:

Information and assistance, long-term care ombuds-man, case management, adult protection, caregiverservices, and congregate and home delivered meals.These services exist in every Area Agency on Aging inIdaho and are covered regularly in this annual insert.

At our Area Agency on Aging, we strive to be theleaders in Idaho, with regard to service delivery. Twoareas which we innovated in the past are ourGrandparents Raising Grandchildren Program and theVolunteer Ombudsmen Improving Care for our Eldersprogram. The former provides a wide array of assis-tance specifically to grandparents and other relativecaregivers of minor children. The VOICE programtrains volunteers to make regular, unannounced visitsto long-term care facilities to observe the quality ofcare, advocate for resident rights, and investigate andresolve problems as they arise.

We are continuing to be leaders of innovation in ouraging network through two new programs. First, wehave partnered with the Community Family Clinic toprovide dental vouchers to seniors who do not havedental insurance. The Community Family Clinic pro-vides services to eligible participants on a sliding feescale. Our vouchers will pay at least a portion of thatfee. For low-income seniors, our vouchers will pay thefee in its entirety.

Another area of innovation is the inclusion of anonline resource database. At the Area Agency onAging’s website — www.eastidahoaging.com — wehave created a link to our resource database, which issearchable. It is our hope that if you have a need, ourdatabase can connect you to a local resource to addressthat need.

With everything that we do, we have great plans forthe future. We are actively working as part of a coali-tion with area hospitals to put systems in place toimprove the well being of patients as they are dis-charged into their own homes. In doing so, we hope tosee a decrease in hospital readmissions and an overallimprovement in the well being of eastern Idaho’s sen-iors.

The last year has flown by. We still encourage peopleto “know your neighbor.”

We are cautiously watching the political landscape asthe battle to protect Medicare, Medicaid and SocialSecurity continues to rage.

Primarily, though, all of our efforts are focusedaround the importance of creating systems to allowseniors to live independently and safely, in their ownhome, for as long as possible.

Health care fraud is not a victimless crime

As shown by recent indictments, Medicare is anattractive target for criminals. Attorney General EricHolder and Health and Human Services (HHS)Secretary Kathleen Sebelius announced that a nation-wide takedown by Medicare Fraud Strike force opera-tions in seven cities has resulted in charges against 107individuals, including doctors, nurses and other licensedmedical professionals.

According to court documents, the defendantsallegedly participated in schemes to submit claims toMedicare for treatments that were medically unneces-sary and oftentimes never provided. These operationsmark the fourth in a series of historic Medicare fraudtakedowns over the past two years.

“Health care fraud is not a victimless crime,” says FBIDeputy Director Sean M. Joyce. “Every person who paysfor health care benefits, every business that pays higherinsurance costs to cover their employees, every taxpayerwho funds Medicare — all are victims. The FBI will con-tinue to work closely with our federal, state and local lawenforcement partners to address health care fraud andabuse. We will use every tool we have to ensure ourhealth care dollars are used to care for the sick — not toline the pockets of criminals.”

Ombudsmenlend their voicesto those who cannot speak forthemselves

Volunteer OmbudsmenImproving Care for ourElders (VOICE) is theAssistant OmbudsmanProgram which recruitsand trains volunteers tovisit nursing and assistedliving facilities inBonneville County on aweekly basis.

The mission of the LongTerm Care OmbudsmanProgram is to provideombudsman services to allresidents of all long-termcare facilities throughadvocacy and education.

The goals of the pro-gram are: � To advocate for resi-

dent rights,� To investigate com-

plaints on behalf of resi-dents and work towardresolution,� To provide communi-

ty education on long-termcare issues,� To provide ombuds-

man services to all resi-dents of all long-term carefacilities in a nine countyarea, (one volunteer toeach of our 40 facilities).

Each volunteer receives24 hours of curriculum and32 hours of one-on-onementoring. Backgroundchecks are mandatory. Atechnical assistance meet-ing is held monthly withupdates and training to thevolunteers. With our volun-teers, facilities are visitedweekly and the residentsknow that they have advo-cates who care and willsolve problems for them.

In our nine-county area,the 2010 census showedthat out of the 202,463 resi-dents, 20,460 were over theage of 65. Of those, 1,172are living in long-term carefacilities. The OmbudsmenProgram provides a forumfor citizens who needadvocacy for long-termcare issues.

Why are seniorstargeted for fraud?

If you are a senior indi-vidual, you might be ask-ing yourself this question:Why are seniors more vul-nerable and more often

targeted than a youngerindividual? Here are somereasons:� They are more likely

to be at home to answerthe door or the telephone.� They tend to be from

an era when people weremore trusting and are lesslikely to spot a con artist.�Often lonely, they are

more susceptible to listento a friendly pitch.� They may be physi-

cally incapable of doingtheir own home repairs,thus making them the per-fect recipient for this typeof scam.� They are targeted for

financial schemes becausethey are more likely to owntheir homes free and clear.

How can you protectyourself from scams andfraud?� First, register your

telephone number with theNational Do Not CallRegistry at (888) 382-1222or their website atwww.donotcall.gov.�Never do business

with strangers who justshow up at your door. Ifyou don’t know them, don’topen your door.�Never give out your

personal or financial infor-mation to an unknowncaller. A rule of thumb is —if you didn’t call them, youdon’t give out your infor-mation.� Always write down

the date, name, telephoneand address of anyone youtalk to, in person or on thetelephone.�Never sign anything

unless you fully under-stand what you are sign-ing. If you are not sure,don’t sign.
