These combine acute care training with several opportunities in academic departments, and therefore excellent opportunities for rapid and comprehensive progress through postgraduate clinical skills and examinations.

What arrangement of day release or block release will be used for protected research time?

ACFs would have access to day-release training on the accredited Postgraduate Certificate in Health Research course run by the University of Leeds as part of their study leave entitlement, preferably before undertaking the dedicated period of research.

The block release 9 month research period would be within the Section of Molecular Gastroenterology (within the Leeds Institute of Molecular Medicine, School of Medicine, University of Leeds) likely consisting of separate 6 month and 3 month periods, the precise timing of which could be determined to fit best with MRCP training commitments and the timing of Research Fellowship applications

Academic Clinical Fellowship Training Programme: Research Component

ACFs choosing Gastroenterology for their dedicated 6 month research training post will be hosted in the Section of Molecular Gastroenterology in LIMM by the Research Training Supervisor, Professor Mark Hull. The ACF will spend one week attached to 5 research Groups covering the spectrum of Gastroenterology research in LIMM (molecular/cell biology of carcinogenesis, mouse models of GI cancer, functional imaging of the GI tract, epidemiology of GI cancer risk, inflammatory bowel disease) in order to experience differing aspects of applied Gastroenterology research. During this time, the ACF, in discussion with the Research Training Supervisor, will choose a topic for a 3000-word literature review dissertation to be completed by the end of the post. It is expected that this literature review could lead to development of a future Clinical Research Training Fellowship proposal. The ACF will then spend 4 months attached to a Research Group of his/her choice in order to gain specific research methodology training. The final month will be spent completing the dissertation leading to development of a Research Fellowship proposal. During the attachment, the ACF will attend a fortnightly Gastroenterology Journal Club, a fortnightly GI research-in-progress meeting in LIMM and will be expected to attend the many excellent outside speaker seminars arranged in LIMM. A further 3 month period of dedicated research time could be used to submit a Research Fellowship application or spend time with a different research team.

Academic Clinical Fellowship Training Programme: Clinical Component

This post can be offered at ST1, ST2 or ST3 level. ACFs planning to enter at ST1 or ST2 level will undergo core medical training co-supervised by the Yorkshire School of Postgraduate Medicine whose remit is to ensure the provision of an education of a consistent high quality across the Yorkshire and Humber Deanery, manage initial

selection, subsequent allocation and appropriate placements. The will also provide induction (in collaboration with the host NHS Trust), appraisals and competence based curriculum delivery assessments. The clinical training programme will be tailored to suit the needs of the trainee and will rotate through academic medical units where the ACFs will complete their core medical training and obtain the MRCP.

NB: Candidates applying to enter at ST3 (direct into specialty) must have already obtained their full MRCP (Part 1, Part 2 & PACES).

Clinical Gastroenterology Training will be based mainly in the Gastroenterology Unit in Leeds General Infirmary (LGI). This is a busy Unit caring for in-patients with the full spectrum of GI and liver disorders, particularly GI bleeding, alcoholic liver disease and inflammatory bowel disease. Out-patient work and Endoscopy are based in all the hospitals of the Leeds Teaching Hospitals NHS Trust including LGI, St James’s University Hospital, Seacroft Hospital and Wharfedale General Hospital. In addition to general Gastroenterology clinics, there are specialist clinics for Coeliac Disease and Inflammatory Bowel Disease, which the ACF will be encouraged to attend, particularly if it is relevant to an academic interest. The ACF will also train in endoscopy dependent on seniority. There are weekly radiology and monthly histopathology meetings. The ACF will experience the full range of diagnostic modalities used in Clinical Gastroenterology, which will be invaluable as the ST ACF decides whether to pursue Specialist ST3 training in Gastroenterology.

This post attracts an NTN(a) with run-through to Specialty Training. For ST1-2 entrants, subject to satisfactory acquisition of CMT competences (including the completion of MRCP exams), the post will run through to ST3-CCT in Gastroenterology/GIM. This will be arranged around out-of-programme training for a doctorate or similar further research training.

Successful candidates: The exit from this post will typically be to an externally-funded research fellowship, leading to award of a PhD and subsequently application either for a Clinical Lecturer post or a Clinician Scientist Fellowship. Obtaining funding for that period of research training beyond the ACF will be supported by senior academics, but success cannot be guaranteed.

Unsuccessful candidates: If the postholder does not achieve the expected clinical competencies, this will be handled in the same way as for all other trainees in speciality medicine.

If the postholder fails to achieve academic competencies, or is unsuccessful in obtaining research funding, they would be anticipated to return to a clinical training post. This will be discussed in good time with the Programme Directors through the system for appraisal and mentoring of academic trainees. Whilst no guarantee of an appropriate post is possible, every effort will be made to accommodate such individuals within the local training schemes.

General Statement of Policy

General Statement of Policy regarding fitness to practice proceedings by a licensing/regulatory body and relating to criminal records.

Registration with a professional body, such as the General Medical Council or General Dental Council, imposes a duty on health care professionals to provide a good standard of medical care for, and behave appropriately towards, patients. NHS employers also have a duty to ensure that patients receive a good standard of medical care and to take all practicable steps to ensure the safety of patients and staff.

In order to assess job related risks, the Yorkshire and Humber Deanery needs to establish if applicants who may be offered a placement on a Specialty training programme have a criminal record in the UK or in another country that they can lawfully be required to disclose.

Applicants for Specialty training posts are exempted from the Rehabilitation of Offenders Act 1974, and can therefore be asked to disclose “spent” convictions that they would otherwise not have to declare.

Applicants for Specialty training posts will also be asked if they are “bound over”, have received a police caution, warning or reprimand and whether they have been charged with a criminal offence that is not yet disposed of.

Prior to making an appointment, the Yorkshire and Humber Deanery also needs to establish if applicants for Specialty training posts have ever been disqualified from the practice of a profession or required to practice it subject to specified limitations following fitness to practise proceedings by a regulatory body in the UK or in another country, and whether they are currently the subject of any investigation or proceedings by any body having regulatory functions in relation to health/social care professionals, including such a regulatory body in another country.

Where such a position involves regular care, training, supervision or sole charge of persons aged under 18 or vulnerable adults (as defined by The Police Act 1997 (Enhanced Criminal Record Certificates)(Protection of Vulnerable Adults) Regulations 2000), applicants will also be asked whether they are currently the subject of any police investigation in the UK or any other country of which they are aware.

Where the position is a “regulated position” under the terms of the Protection of Children Act 1999 (as amended by the Criminal Justice and Court Services Act 2000), checks will be carried out in accordance with the Protection of Children Act 1999. Anyone whose name is on the list is legally barred from working with children. It is an offence for such a person to knowingly apply for, offer to do, accept or to do such work. If you are appointed to a Specialty training post, you will be subject to a Criminal Records Bureau (CRB) check.

Applicants are encouraged to declare criminal conviction information and any other matters that are, or that might be, relevant to the position for which they are applying at an early stage in the appointment process.

All applicants are also expect to inform the Yorkshire and Humber Deanery if, before taking up any position offered to them, they are cautioned or charged with a criminal offence in the UK or another country.
