Application How much does the tournament cost for my team? The cost per team by age group can be found on the main page of our Fort Wayne Invitational Tournament webpage www.fwisoccer.com and also is listed in the Permission to Host / Sanction Form. If you have any questions, please contact the tournament director, Heather Bokhart at [email protected] Can I check on the status of my team’s application? You can check your team status by logging on to www.gotsoccer.com from the Fort Wayne Invitational webpage www.fwisoccer.com. You will need the name and password that you provided when registering for the tournament. If you have any questions, please contact the tournament director, Heather Bokhart at [email protected] Do I send a copy of my team roster or permission to travel? You will provide a copy of your team roster and a copy of the State’s Permission to Travel when you check in your team at registration on Friday June 18, 2010 at the host hotel – Holiday Inn. This year we have added the option of paying by credit card when applying to the tournament. Otherwise, you may continue to pay by check, making your check payable to Citadel FC and mailing your check to the tournament director:

What hotel may I choose for my team to stay in? All teams are required to book hotel rooms using the “Hotel” link on the Fort Wayne Invitational webpage www.fwisoccer.com Teams not staying at one or more of these properties will not be considered for acceptance to the tournament. While you are free to select any hotel property you wish, you will be asked to VERIFY your rooming arrangements when you register your team on Friday night. The Fort Wayne Invitational Tournament Committee and Citadel FC assume no liability by its endorsement of a particular hotel. How do I find out which field my team will play on? We do not have designated fields for each division. The fields are determined after we accept the teams, based on the number of teams per division and the available fields. Kreager Park has both small-sided and full sized fields. Please continue to check the website fwisoccer.com for schedules and field assignments.


Registration When and where is Team Registration for the 2010 Fort Wayne Invitational? Team registration will be Friday June 18, 2010 at the host hotel:

(260)482-3800 Does my team need to register on Friday night if our first game is at 10:00 AM Saturday? Yes – all teams will need to register on Friday June 18, 2010. I need duplicate forms, how do I get them? All forms will be on the Fort Wayne Invitational webpage www.fwisoccer.com What do I need to bring to the team registration? Please bring the following to team registration on Friday June 18, 2010

• Original state approved roster and one copy of the roster to leave with tournament staff • Validated player pass cards for both rostered and guest players • Validated coaches pass cards, one for each coach and manager (maximum of three) • Tournament liability form which must be signed by all players and parent/guardian • Travel permit if traveling from outside of Indiana • Guest player form

You will also be asked to fill out the Team Contact Form with team contact information including cell phone numbers and the name of the hotel you are staying with while attending the tournament. When is my team roster frozen? Team rosters will be frozen at registration on Friday June 18, 2010.
