MORE THAN THREE years afterHurricane Katrina crippled theNew Orleans area’s health carecommunity, many in the industryfeel there’s been little progresstoward recovery.
Many professionals are stillchallenged by a shortage of healthcare workers, while others aredeeply concerned about the lackof attention to preventive healthcare, be it because of poor access
or financial strains.The overriding concern con-
tinues to be sharply decreasedaccess to the Charity health caresystem, which has caused anincrease in uninsured patients vis-iting other facilities that are nowin a financial crisis because ofslow Medicaid and Medicarereimbursements.
There are bright spots, how-ever.
Several specialty facilities andprograms have opened in the pastyear, and the return of a majoracute care facility — OchsnerBaptist Medical Center — hastaken the pressure off of nearbyfacilities.
Most medical specialists point-ed to new techniques and technol-ogy as the biggest advancement intheir respective fields, helpingimprove the accuracy of treatments
and procedures.But we still have far to go.Decisions have been made
regarding a new Charity hospital,and while plans are still on thetable for a downtown medicalcorridor, including the NewOrleans BioInnovation Center,both projects are still years offfrom completion.
The longer the area lacksaccess to a Charity system tosupport the region’s uninsured,the harder our hospitals will fallfinancially.
We can only hope our healthcare providers remain committedto the area long enough to ride outthe recovery and see the systemfunction properly.•

No. 1 concern:Skin care and skin cancer awareness. A lot ofpeople think all dermatologists deal with isacne, but we deal with a lot more importantthings. Dermatologists also want to makesure people are asking about certification.That’s been a challenge for the area, with theeconomy what it is and insurance companiescutting back. A lot of doctors are doingthings that they really weren’t trained in.
Brightest spot:Psoriasis treatment has made someadvances. We’re netted into a newer genera-tion of treat modalities, and these are thenewest and the latest treatments. It’s sort of aguided missile treatment as opposed to ashotgun treatment.
Return to normal:You may get some variations on this theme,but I’m busier since the hurricane. We had alarge volume of people who moved over here
(to St. Tammany Parish). And talking to mypeers in New Orleans, I think dermatology’sdoing pretty well.

No. 1 concern:We’re all very concerned because all ofour emergency rooms are clogged up.One of the concerns we have is forheart attack and stroke. People are hav-ing access problems, and to do preven-tive care you have to have open accessand good primary care. People areunder stress, and people who arestruggling to keep their home togetherdon’t have time to worry about theircholesterol as much as they should,perhaps smoking is more than itshould be and perhaps alcohol use ishigher than it should be.
Brightest spot:I want to make sure I don’t think likean Ochsner doctor, but we just openedthe emergency room at Ochsner
Baptist. That’s a big deal because thatdecompresses the community betweenTouro and our main campus. We’vealso opened an open-heart surgeryprogram at Meadowcrest, which alsohelps the West Bank significantly.
Return to normal:We’re missing a whole subset of healthcare workers, which include what wecall the middle class. We’re missingtons of nurses and technicians. Wehave tons of beds, but we can’t staffthem. If we had more nurses, we’d hirethem in a heartbeat. To get back tonormal, we would have an influx orreturn of the middle class of workers,particularly those nurses and techni-cians. We have enough doctors; weneed more nurses.

No. 1 concern:I see people with all kinds of infec-tions, but the majority of patients I seehave HIV or AIDS. They make up thebiggest part of infectious diseases. Thefact that many people are still not get-ting tested is a huge concern. We needto make it more available, more rou-tine. Another concern is the difficultiespatients have adhering to their medi-cine routines.
Brightest spot:We can treat HIV so effectively nowthat our patients can essentially livenormal lives. The people with HIVnow who take their medications canlive almost as long as someone without
HIV. I tell my patients if you take yourmedicine, you’ll be great, and if youdon’t, things can happen.
Return to normal:Locally, numbers-wise, we’re doingwell as far as infectious disease special-ists. Very few of them left after the hur-ricane. At the same time, it’s interestingthat most of our patients returned tous. On the whole, HIV services aredoing fairly well. I think the city wasremiss in not getting some of the appli-cations in for government support ofHIV patients. Every year there’s anopportunity to apply for federal fund-ing to help us assist and provide for thespecial services of HIV patients.

Diseases caused by a virus or bacteria that can be spread from one living thingto another such as HIV, meningitis, malaria, strep throat and influenza
No. 1 concern:Health insurance. As far as prescribing medi-cine, it dictates what medicine can be insuredand which medicines are authorized. It makesit difficult because you have treatment plansyou want to do. You want to order the testsand medicines you think are the most impor-tant, and you get call backs from the insurancecompanies telling you this isn’t authorized oryou have to take additional steps.
Brightest spot:There’s more preventative health care test-ing. People are keeping up with mammo-grams, colon cancer screenings and treat-ment for diabetes. It’s never as good as wehope for it to be, but it has improved.
Return to normal:We’re recruiting at our own practice and arealways looking for people who have ties tothe area, but it’s difficult to keep people inthe practice in this area. I think a lot of peo-
ple are leaving just because of the state ofhealth care in the area. Many people feel thestate and federal government aren’t bringingin the funds to help us out.

No. 1 concern:We’re concerned about the inadequate supplyof health care workers who are trained to pro-vide hemodialysis or who are willing to betrained to provide hemodialysis in hospitalsand dialysis clinics. Many of the trained patientcare technicians who left the area with Katrinaeasily found jobs in every community becausethere’s a nationwide need for dialysis workers.It takes six to 12 months of mentoring to fullytrain a technician until they become proficient.
Brightest spot:Both Tulane and Ochsner have fully func-tional kidney transplant programs and havebeen able to recruit additional specializedsurgeons and keep them in the region.
Return to normal:Our region has very high rates of kidney dis-ease. There has been a very slow return of pre-
ventive care services to the general populationwhere kidney disease can be diagnosed earlyand the diseases that contribute to kidney fail-ure can be managed intensively so that our
population has improved kidney health,specifically the network of community clinicsavailable for the uninsured, under insured andtransient members of the population.

No. 1 concern:To ensure we have a sufficient number ofprofessionals and work force to take care ofour future generation. I don’t think we haveenough at this point in time. We need to belooking ahead and be a lot more aggressivein ensuring there are enough nurses.
Brightest spot:We are all enjoying the availability of thegreat technology and innovative ways weapproach nursing and patient care. Our bed-side reporting — communication betweenoncoming and outgoing nurses at thepatient’s bedside — has improved greatly.
Return to normal:We need to look forward to the challengesthat are coming up, but what we need to do
is be ready for whatever is coming our way.Health care changes every day. It’s never the
same and we have to keep looking ahead atwhat our challenges are for the future.

No. 1 concern:The burden of dealing with theinsurance issue. It’s become moreand more burdensome to takecare of the patients with endlessdenials from insurance compa-nies. The other thing is we’ve hada lot more indigent patientsbecause of the Charity situation.We aren’t really any better off thanright after the storm. The otherthing is the patients in the Charitysystems were not really followedvery well.
Brightest spot:We have some very good hospi-tals here and some very goodphysicians. With the availabilityof all the new technology, not
only in medical oncology butradiation oncology, things havechanged greatly over the past 10years. In terms of cancer, you canget very good care in the city ofNew Orleans.
Return to normal:I think it would help if they couldget some kind of workable Charitysystem up. It’s been a big burdenand cost millions of dollars, andthe sad thing is the governmenthas not put up the money. Thesehospitals are in dire straits, andsome of those millions in debt arefrom taking care of those patients.Keeping our hospitals in a veryhealthy condition and compensat-ing for that care is a priority.

No. 1 concern:There’s been a big focus on obesity in chil-dren. That’s something we wrestle with inNew Orleans as well. We want to reframepeople’s expectations when it comes to ahealthy way to eat. We also want to make surevaccines are up to date. That’s been particu-larly challenging after Katrina wiped out a lotof records. It’s also just difficult to make surepeople are coming in at the right times forvaccines.
Brightest spot:A bright spot for us is seeing people contin-uing to come back, continuing to recover.
Return to normal:The Vietnamese community that Kids FirstNew Orleans East serves has returned to apre-Katrina status. This clinic wasn’t here
pre-Katrina. The mental health area isunderserved, and pediatricians need to makereferrals. Children’s Hospital is also pushing
for electronic medical records, and I thinkthat is going to help the community enor-mously.

No. 1 concern:I know there’s a problem withaccess to care. There’s a shortage ofpsychiatrists in New Orleans sinceKatrina. Patients with psychiatricdifficulties might not be able to beadequately treated, mostly becauseof geographic obstacles. The wait-ing time to see people in my clinic ishigher than it was before the storm.
Brightest spot:As an LSU psychiatrist, we got ourhospital reopened and our clinicsare expanding. The fact that we’reputting things back together post-Katrina is just wonderful.
Return to normal:People were dissatisfied withaspects of the previous system, sowe wouldn’t want to necessarilybring it back exactly as it was. Weneed closer linkages with publicaccess units and public psychiatrichospitals, and we need to coordi-nate psychiatric treatment with lawenforcement. We’ve had a drasticreduction of psychiatric beds inNew Orleans, and therefore wehave to work harder to keep peoplefrom needing hospitalization.

No. 1 concern:There’s more work than there are pulmonolo-gists coming out of training. It’s getting worse,there’s more need for pulmonary care andthere are fewer physicians.
Brightest spot:What’s making my job easier is the technol-ogy — computer-based radiographs, all thetechnology in the ICU.
Return to normal:The problem with attracting physicians isprobably salary. The salaries in New Orleans
are below what they would be. On the NorthShore, since about a year after the hurricane,we’ve returned to a pre-Katrina status. Four
years later, we’re about 20 percent to 30 per-cent better. We have more resources now,more patients now.

No. 1 concern:The concern would be loss of revenuebecause of competition from specific hospi-tals. Another concern is the lowering of reim-bursements we get from the federal govern-ment for government-sponsored programssuch as Medicare and Medicaid.
Brightest spot:Technology. The entire industry continues tochange. Here at this hospital, we have gonefrom film to digital, meaning the radiologistsare reading (things like X-rays through) com-puterized images. We also have a 64-SliceCT scanner, which displays images fastenough to show the beating heart.
Return to normal:After Katrina, we actually saw an increase inour population in Slidell. We’re starting to
see the hospital go down to a more normallevel. The months where the doctors weregone and the patients were gone, you’re
never going to make up that revenue. But itwas a much shorter recovery than the NewOrleans area faced.

No. 1 concern:Declining reimbursement, particularly in light ofthe fact that many procedures we do are high riskand are more likely to lead to possible malpracticeissues. Because of the nature of the diseases wetreat (chronic kidney disease), most of ourpatients are covered by Medicare or Medicaid.
Brightest spot:We have developed many new techniques todecrease the risks in treating diseases. A goodexample is treating abdominal aneurysmswith stent grafts. The patient gets two smallincisions in the groin and an overnight stayrather than a long abdominal incision and aweek in the hospital.
Return to normal:With population shifts and the decreasedaccess the Charity system, we continue to see
many more patients with poor or no healthinsurance coverage. I think we have a longway to go to improve coverage so hospitalsand physicians will be able to afford to carefor patients previously cared for in the
Charity system. I’m not convinced theCharity system as it existed before will everreturn, and I am also not convinced that theproposed “medical home” health care systemwill allow patients access to specialty care.
