Patients and families are the guiding force in their own end-of-life experience.

Dear Friends,

From time to time, a responsible organization must review and

revise its mission, vision, and values to accurately represent its

services and the needs of its constituents. During this past year,

Hospice of Santa Cruz County’s Board of Directors and executive staff

undertook this important task. The resulting intentions shown on the cover of this

report emphasize a long-held ideal—that patients and families are the driving force

behind their end-of-life decisions. This concept has been, and will continue to be,

foundational to every aspect of HSCC’s programming as we strive to honor the wishes

of our community members with the utmost integrity and professionalism.

HSCC’s dedicated staff and volunteers provided a record breaking 46,726 days

of hospice care during 2009/2010 while maintaining performance standards that

have surpassed state and national benchmarks. Stringent MediCare guidelines for

participation were met, thereby securing 90% of our annual budget through this

essential revenue stream.

The impact of our Transitions program has also been profoundly felt in the

community with HSCC’s medical social workers easing the path from the hospital

to home for more than 350 individuals annually. Grief support and educational

programs are thriving, and we continue to focus on expanding bilingual access to

these vital services.

While challenging economic times persist, I am happy to report that Hospice of

Santa Cruz County continued to serve every eligible patient regardless of their ability

to pay. Charity care for the uninsured in the amount of $90,000 was generously

underwritten entirely by community members during the year. HSCC also realized

a $273,240 savings as a result of the more than 13,000 hours of program support

provided by volunteers.

Our successes in 2009/10 bring to mind Helen Keller’s words, “Alone you can do so

little; together we can do so much.” We at HSCC do not fulfill our mission alone.

I am deeply indebted to the broad family of supporters, philanthropic foundations,

Legacy Circle members, Friends of Hospice, Hospice Angels and volunteers who

have made our work possible in the past, who continue to do so today, and who

hold the vision of compassionate end-of-life care for our future.

An Alternative to Being AloneGrief Support Grief is a quiet suffering, a challenging journey that can begin prior to the death of a loved

one and can continue long after they are gone. Through HSCC’s grief support program,

individuals are encouraged to embrace loss as an important aspect of life. Healing sessions

for the bereaved take place privately or in groups and are age-appropriate. Support is provided

in English or Spanish and is available to all Hospice families and to the community at large.

• 1,822 Hospice family members and 336 community members assisted through the grief

process

• 833 grief support sessions provided for children and teens

• On-campus bilingual teen grief groups received national recognition for innovation

• Weekly drop-in groups, the Interfaith Memorial Service and the Grief & the Holidays

workshop address the year-round need for support

Self-Determination is an OptionCommunity EducationEvery day we make decisions that affect how we live, define our values and give voice to our wishes. At HSCC, we believe that life’s end should similarly be a time of opportunity and self-empowerment. Our educational workshops and clinics are designed to provide valuable information about end-of-life choices.

• Led the county-wide initiative to make the Physician Orders for Life-Sustaining Treatment form (POLST) a new standard of care in Santa Cruz County• Facilitated 85 educational presentations providing 2,500 professionals and community

members with end-of-life education

• Collaborated with eight community agencies to develop an innovative Caregiver Certification Course through Cabrillo College Extension

Throughout the year, Hospice of Santa Cruz County programs are touching the lives of families in need, meeting them where they are in their end-of-life experience, in their grief and loss, and in their need to plan for the future.

Quality of Life Choice Hospice CareComfort-oriented in nature, HSCC’s hospice program aims to provide a

meaningful quality of life for people living through the final months of a terminal

illness. Expert care focuses on controlling a patient’s pain and symptoms and on

helping family and friends cope with the stress and practical needs of caring for

their loved one.

• Provided 46,726 days of hospice care to 762 patients and their loved ones

• Delivered 401 days of charity hospice care to the uninsured

• Bilingual and bicultural care teams ensure that our Latino community has access

to our services.

Preference for Planning Transitions The life’s-end journey can be overwhelming, especially when one is faced with the

daunting prospect of debilitating illness. For those individuals who do not feel ready

for hospice, or who are medically inappropriate for the service, Transitions is often

the help they need—support to sort out the practical and emotional considerations of

their diagnosis, guidance with decision making, help connecting with resources, and

professional assistance with documenting their end-of-life wishes.

• Served 358 individuals and their caregivers

• Collaborated with more than 50 community-based agencies to provide care

• Initiated Saturday hours to accommodate the needs of out-of-town family members
