The widespread civil strife that broke out in the aftermath of Kenya’s December 27, 2007 presidential election was unexpected and violent. Protests sparked by disputed election results and allegations of vote-counting inpropriety gave way to outbreaks of violence along political and then tribal lines.

Exact numbers have been hard to calculate, but the Kenyan Red Cross estimates that more than 1,000 people were killed, and UN offi cials estimate that 500,000 people were displaced throughout the country. Political reconciliation began in the form of a new coalition government agreed to on February 28, the details of which are still being negotiated.

To listen to Dr. Hezron Mc’Obewa, director of the Kenya-based OGRA Foundation and a Direct Relief regional medical adviser, is to discover hope for the people of Kenya. Despite being engulfed in the eye of the violence and having his personal offi ce burned to the ground, Dr. Mc’Obewa pressed on to aid people from all tribal groups caught in this crossfi re.

Educated in England, Dr. Mc’Obewa helped start OGRA Foundation as a community-based organization to provide health care and promote socio-economic and cultural development. Because of OGRA’s strong community reputation, Dr. Mc’Obewa was appointed as the head of medical outreach for the Kisumu area’s ad-hoc humanitarian aid committee, coordinating between aid agencies, governments, and international bodies.

Despite communication issues that arose from the instability created by the violence, Dr. Mc’Obewa was able to contact Direct Relief the day violence broke out. Direct Relief immediately wired $10,000 to purchase needed medicines and fuel, set in

motion airfreight shipments of additional supplies, and pledged to provide over 136,000 courses of antiretroviral medicine for patients with HIV whose treatment was disrupted by the fi ghting.

As the scope of the violence grew, so did Direct Relief ’s assistance. On top of the airfreighted provisions, Direct Relief contributed $100,000 in emergency funds to help transport fl eeing displaced families, to provide essential medicines and drugs, for monetary support of surgeons, doctors and nurses, as well as for fuel costs and other contingencies.

Throughout the two months of intense confl ict,

Dr. Mc’Obewa maintained regular contact, often via text messaging, the only available means. In a chilling entry from mid-January, he described a team of OGRA staff going door-to-door, looking for injured people who were unable or afraid to come outside and seek treatment. “It is through this that we are encountering some people with, literally, rotting limbs – how some haven’t died, only God knows,” he wrote.

OGRA and Dr. Mc’Obewa’s assistance went beyond medical. On the morning of December 31, about 300 youth armed with machetes, protesting the presidential results, attacked Grace and Habel Odongo at the guest houses they run in Kisumu.

Before fl eeing, they sent a distress signal to OGRA, who arrived in time to take them 14 miles away to safety and later to an evening fl ight out of Kisumu.

OGRA has handed over the day-to-day running of health camps and clinics to the Kenyan Ministry of Health, but Dr. Mc’Obewa and his staff are still looking for ways to improve community cohesion and provide assistance to the displaced. Direct Relief will be there to help them.

As daunting as these statistics are, the day-to-day realities of living with diabetes in an area without adequate care are far worse. Fortunately, the many health complications related to diabetes can be mini-mized or eliminated entirely through early detection and changes in daily lifestyle.

Dr. Elizabeth Duarte Gomez confronted the crisis in Bolivia – where 4.8% of the population is diabetic (IDF, 2003) – when she founded the nonprofi t El Centro Vivir Con Diabetes (CVCD). Dr. Duarte, an endocrinologist trained in Spain, placed the organization at ground zero of the disease, in the city of Cochabamba, where CVCD estimates 9.4% of adults are suffering from diabetes.

For seven years, the CVCD clinic has been at the forefront of diabetic support in Bolivia, focusing on lifestyle education and nutritional counseling along with providing treatment for the most common diseases that accompany diabetes. By providing extensive health education and promoting healthy eating habits, the clinic’s small but dedicated staff works against the lifestyle trends that increase the incidence of diabetes. The clinic’s outreach services

strive for early detection and diagnosis, and its main clinic provides complimentary treatment for those who have developed related visual, neural, and circulatory problems.

Direct Relief has supported CVCD since its inception with primary care medicines and medical supplies that aid the treatment of diabetes-related conditions. Abbott has come to CVCD’s aid with blood glucose meters and test strips critical to early detection and monitoring, allowing for control of the disease through regular clinic visits and education. The company’s philanthropic foundation has also provided cash grants to bolster the clinic’s outreach services.

With this help, CVCD has gone mobile. Over 13,000 people have been screened for diabetes in eight of the nine major Bolivian cities by clinic staff in the last four years. Of those screened, CVCD discovered that 7.9% had previously undiagnosed cases of diabetes. Those

diagnosed learned from the outset how to properly manage their diabetes, and by living healthier lives, they will impact less an already fi nancially strapped public health system.

In addition to screenings, CVCD has distributed printed materials explaining diabetes management, con-ducted group and individual disease education using Abbott-contributed

glucose meters and strips, and trained 604 health professionals (doctors, nurses, and pharmacists) on the latest diabetes detection and treatment methods.

When Damon Taugher, Direct Relief ’s Director of Domestic Initiatives, delivered a presentation to the National Association of Community Health Centers’ (NACHC) annual Policy & Issues Forum in Washington, D.C. this past March, he had two purposes.

The fi rst was to discuss Direct Relief ’s response to the wildfi res that burned throughout California in late 2007. Along with colleagues from the California Primary Care Association and Mountain Health and Community Services in San Diego County – a clinic that became a

shelter and a staging point for various emergency services during the wildfi res– he covered the benefi ts of partnerships that helped Direct Relief deliver medicine, medical supplies, and fi nancial assistance to clinics and shelters during the weeks-long evacuation period.

*Cash assistance was targeted at nonprofi t clinics caring for low-income patients. 17 clinics in San Diego County that care for 800,000 patients annually were granted $515,000 to assist with physical fi re damage, mobile clinics, staffi ng increases, and overtime. $50,000 was provided to the CDF Firefi ghters Benevolent Fund to support the recovery of injured fi refi ghters.

Those partnerships also enabled rapid fi nancial assistance ($565,000 thus far) to help clinics quickly recoup signifi cant fi nancial losses as they suffered from fi re damage, increased patient loads, site closures, and lost revenue.

But Taugher wasn’t there just to talk about past performance. As he sees it, the same principles and systems used to connect Direct Relief and other groups to the Golden State’s network of NACHC members could also be used to create a template for regionally-appropriate emergency preparedness and response nationwide.

“Creating strong relationships prior to disasters is crucial for effi cient coordination in post-disaster situations,” said Taugher. “These partnerships allow for critical resources to reach people most often at risk.”

Partnering with NACHC gives Direct Relief access to the largest association of nonprofi t health centers in the United States. Between its 7,000 health centers and clinics, NACHC members provide services to roughly 16 million patients a year, approximately 41 percent of whom are uninsured.

This broad reach, especially among uninsured Americans, is also a critical benefi t for the partnership’s other aim: getting medicines and supplies into the hands of nonprofi t health centers and clinics that treat patients without healthcare coverage. Working with NACHC, its state-level counterpart associations, and individual clinics, Direct Relief has delivered $125 million in medicines for working-poor, uninsured patients nationwide since 2004.

MR. JOSEPH R. KIRCHMAIER, a volunteer with his wife Edythe at Direct Relief International for forty years, passed away peacefully at home surrounded by his loving family on January 28, 2008. Joe and Edythe moved to Santa Barbara in 1938. During World War II, Joe served as the Red Cross Field Director at Camp Cook. Following the war, Joe served as the Social Services Supervisor and Admissions Offi cer at the County General Hospital. In the 1970’s Joe and Edythe made three missionary trips to Taiwan to teach English and social work at Tianan Theological College. In addition to his family, Joe loved growing cymbidium orchids and stag-horn ferns, as well as cooking and baking. Direct Relief is deeply honored to have the support of Joe and Edythe for the past 40 years. We will miss his deep and personal involvement greatly.

MR. MARCUS E. CRAHAN, JR., a fourth generation Angelino, relocated to Santa Barbara in 1996, and passed away on February 29, 2008. Following his admittance to the State Bar of California in 1957, Mr. Crahan attained distinguished membership in many professional associations including the American Bar, the Los Angeles County Bar, and the Los Angeles Business Trial Lawyers. He was a life-long member of the First Century Families of Los Angeles. Mr. Crahan was instrumental in establishing the Julia Stearns Dockweiler Foundation, a family charitable foundation that provides funding for many worthy non-profi t organizations, including Direct Relief International. His absence will be felt profoundly by the many people his life touched.

The Santa Barbara County Vintners’ Foundation

sponsored the American Riviera Wine Auction on March 14 & 15, 2008, to benefi t the work of Direct Relief. The net proceeds from the silent

and live auctions will provide medical assistance to Direct Relief ’s partners around the globe. This

year, the two evening events raised over $300,000, bringing the total support from the Vintners’

Foundation to $1.5 million.

BOZILOVIC-FIRESTONE WEDDING

Direct Relief would like to express congratulations to Ivana Bozilovic and former ABC “Bachelor” Andrew Firestone on their upcoming nuptials, as well as our gratitude for their extraordinary generosity in asking their wedding guests to donate to us in lieu of traditional wedding gifts. Direct Relief has been an important part of the Firestone Family ever since Andrew’s mother Kate joined the Board of Directors seven years ago.
