Introduction
The NIDA/SAMHSA Blending Initiative was developed in 2001 with the goal of melding science and practice together to improve substance use disorder treatment. The first years of the project focused on the work of "Blending Teams" comprised of staff from SAMHSA/CSAT's Addiction Technology Transfer Center (ATTC) Network and researchers and community treatment providers from NIDA's Clinical Trials Network (CTN). NIDA and CSAT originally convened five separate Blending Teams to develop products that would facilitate the dissemination of research results identified by NIDA as follows: Buprenorphine Treatment: A Training for Multidisciplinary Addiction Professionals (Buprenorphine Treatment), Treatment Planning MATRS: Utilizing the Addiction Severity Index (ASI) to Make Data Collection Useful (Treatment Planning), Motivational Interviewing Assessment: Supervisory Tools for Enhancing Proficiency (MIA: STEP), Short-Term Opioid Withdrawal Using Buprenorphine: Findings and Strategies from a NIDA Clinical Trials Network Study (Buprenorphine Detoxification) and Promoting Awareness of Motivational Incentives (PAMI). All ATTC Regional Centers participated on one of the five Blending Teams. Although the National Office was not directly a part of any Blending Team, as the coordinating center of the ATTC Network, it facilitated the efforts of all five Blending Teams and continues to develop special projects, such as The Training Point, which support the Initiative as a whole.
All five original Blending Teams currently have products in dissemination. One Team, Treatment Planning MATRS, has spawned a sequel that will supplement the original package by including information and resources on the ASI Treatment Planning Software. A new Blending Team related to Buprenorphine/Naloxone-Facilitated Rehabilitation for Heroin Addicted Adolescents/Young Adults held its first “hand-off” meeting in February 2008. Plans for content are still under development; however it is likely that the Team will be assigned to update the existing Buprenorphine Awareness product and to include a new module on using buprenorphine with young adults. In addition, ATTC staff have been working with NIDA on its Blending Initiative task force. They are in the process of developing materials that discuss how to implement evidence-based practices.
Highlights from this Reporting Period MIA:STEP training-of-trainers event held in conjunction with the 2008 Blending
Conference. PAMI training-of-trainers event held in conjunction with the 2008 Blending
Conference. ATTC Blending Initiative Sub-committee formed to update and enhance the
pages related to the Blending Initiative on the new ATTC Network website. The Science of Treatment CD set printed and disseminated nation-wide.

Pre-training/Infrastructure Development Events/Activities
The ATTC Network has found the need to prime the field on the evidence-based practices presented in the Blending packages before actually disseminating the official products. This pre-training/infrastructure development work involves activities such as meetings with SSAs, community treatment programs and other key stakeholders, marketing efforts and product dissemination, foundational trainings, environmental scans and other needs assessments, and overview presentations. From April through June 2008, the ATTC Network reported holding 13 meetings, 2 conferences, 4 technical assistance events, 19 educational events (trainings or academic courses), one product dissemination effort and 2 formal presentations related to pre-training/infrastructure development activities. These efforts are displayed on the map on page 4 and are listed in detail in Appendix A.

Official Blending Product Trainings
Once Blending Teams fully complete the development of their packages and receive final approval from NIDA and SAMHSA, they begin disseminating their packages to the public. All five original Teams now have products in dissemination. ATTC Regional Centers work closely with key stakeholders to find opportunities to deliver trainings on Blending products across the nation. During this reporting period, ATTCs held 60 official Blending product events. Although no pilots were held, several training-of-trainer events were held, including one for PAMI and one for MIA:STEP held in conjunction with the 2008 NIDA Blending Conference. In addition, the Pacific Southwest ATTC held a PAMI training in collaboration with the Pacific Node of the CTN Network during the quarter. Please see the tables on pages 7-8 and 9-11 for summaries of the official trainings held, the number of contact hours they represent and the number of participants served. The first table sorts this information by Blending product while the second table sorts it by Regional Center. These efforts are also displayed on the map on page 9 and are listed in detail in Appendix B.
Several Regional Centers have encountered challenges to delivering one or more of the Blending product trainings in certain states. In such cases, the ATTC has developed innovative and creative strategies to overcome barriers and implement the trainings. See the individual ATTC reports found later in this document for more details regarding challenges and strategies taken to overcome them.

Post-training/Follow-up Activities
Research is constantly contributing new knowledge to the substance use disorder and recovery services field. Yet this new knowledge often is not used in service settings. Implementing changes based on research can be difficult for treatment agencies and professionals. Technology transfer involves creating a mechanism by which a desired change is accepted, incorporated and reinforced at all levels of an organization or system. Although training is one strategy in the technology transfer “tool box,” technology transfer’s scope is much broader than training alone.
The ATTC Network is specifically charged with accelerating the pace of research to practice by elevating the capacity of the substance abuse treatment workforce to understand and adopt cutting edge treatment practices that provide sound and effective treatments for individuals suffering from addiction. The Network’s vision is to “unify science, education and services to transform lives.” Technology transfer is at the heart of all ATTC work.
The ATTC Network utilizes its expertise in technology transfer to extend the reach of the Blending Initiative by re-packaging the approved information and materials in creative and innovative ways. ATTC Regional Centers create products to supplement and enhance the Blending products and conduct follow-up activities and ongoing technical assistance with participants of Blending product trainings to increase the likelihood that research-based practices will be implemented. Follow-up activities are displayed on the map on page 10 and are listed in detail in Appendix C.

The CBHATTC has needed to implement an extensive infrastructure development process prior to implementing the official blending trainings in order to persuade providers to develop skills, gain additional knowledge and change attitudes. b. The CBHATTC uses e-mail communications, faxes, telephone calls and face to face encounters as strategies to disseminate information regarding blending products among governmental agencies, CBOs, and private service contractors. The Mental Health and Anti Addiction Service Administration (ASSMCA) open the first treatment center to opioid dependence patients with Buprenorphine. ASSMCA and La Fondita de Jesús (CBO currently referring clients to this treatment center) service providers need to increase skills in this treatment modality.
This initiative begins with petition of service providers. The challenge was to familiarize supervisors and administrators with service providers’ needs throughout face to face meetings and coordination support.

Saint Croix and Saint Thomas, VI
Introduce MI Approach, Buprenorphine and MATRS Treatment Planning to service providers in USVI state agency and its CBO’s.
Strategies included face to face meeting with Mental Health Acting Director in USVI, identify contact persons in the agency, and establish communication lines by e-mail, fax, telephone calls and face to face encounters to facilitate the recruitment of substance abuse treatment service providers in the State Agencies and CBO’S.

Governmental agencies, private service contractors and CBOs participated in the trainings given on May 15-16, 2008 (San Juan), May 22-23, 2008 (Ponce), June 5-6, 2008 (San Juan) and June 12-13, 2008 (Mayaguez). The booster sessions began on June 23, 2008 in Ponce.
(3) Liaison Activities with CTN Nodes; CTPs; and /or NIDA Researchers a. The CBHATTC, as part of the CTN (Florida Node following the BSFT protocol),
participates in national conference calls relevant to this specific protocol as well as Florida Node conference calls. The lead NIDA investigator for this protocol is Dr. Mike Robbins. Another NIDA researcher who participates in this protocol is Dr. Varda Shoham. Dr. Shoham also served as the principal investigator for a NIDA funded research project that is using Brief Strategic Family Therapy (BSFT) as their platform.
b. NIDA Liaison Coordinator attended the NIDA Blending Conference in Cincinnati, Ohio on June 2-3, 2008.
(4) Development of Ancillary Products This quarter CBHATTC developed a libretto of the Motivational Interviewing
Demonstration in Spanish to be disseminated in DVD format. The libretto is currently undergoing a revision process to evaluate the adherence to the MI Approach.
The CBHATTC is reproducing the MIA:STEP Curriculum in Spanish.
(5) Other One person from the CBHATTC participated in the PAMI TOT (June 4, 08) and
MIA:STEP TOT (June 4-6, 08) in Cincinnati, Ohio.

Their regional program structure has been transitioning to Health Districts, and manpower has been reduced, and yet the demand for services has increased. They think that GCATTC should continue contact with other state programs and Universities. We have been able to provide some TA, but training demand has dropped.

In addition, packaging of the new treatment planning software is being designed which includes new artwork. The Mid America ATTC staff is coordinating the packaging and artwork with the National ATTC office, CSAT, and NIDA representatives. The Mountain West ATTC and the Gulf Coast ATTCs will be assisting in editing the new product instructions along with providing feedback on packaging and artwork as well as, financial support for these new blending activities. Conducted a two-hour presentation at the Tennessee Advanced School on Addictions Tuesday, May 27th 7-9 pm as a pilot test of this product with 40 attendees. The Using Readiness Rulers RTP includes the Mid-Atlantic ATTC readiness ruler (previously developed ancillary product); two video demonstrations (adult and adolescent) using the ruler; and the transcripts for the sessions. The purpose of the RTP series is to address one of the field’s biggest concerns, “I don’t have the time,” by facilitating ease of use and fidelity of the EBP (MI). The product series is entitled: Ready To Practice Innovative Tools for the Active Practitioner. Feedback from pilot test participants

emphasized enhancing the packaging of this product, making it more appealing. The Using Readiness Rulers RTP was also used as an instructional tool during an Advanced MI event conducted at the TN Advanced School.
b. www.motivationalinterview.org :This Mid-Atlantic ATTC website, assisted with funds from the NIDA grant, has benefited awareness of and distribution efforts of MIA:STEP, possibly more than any other single source. This MI website is connected to the Motivational Interviewing Network of Trainers (MINT), and the association has benefited dissemination efforts. Since being featured on the website June, 2007, MIA:STEP has been accessed/downloaded approximately 640,000 times (currently averaging 70,000 downloads per month). It is far and away the most popular resource on the MI site, running at about ten times the rate of TIP35 downloads. Access to the product was enhanced on the MINT home page by converting the section into one document for ease of downloads, so we do not have the specific hits for each section of the product. This investment has proven its usefulness for dissemination to the field of practitioners most likely to promote its use, MINT trainers and those interested in MI.
(5) OtherI. Denise Hall served as the ATTC trainer for third MIA:STEP TOT held in conjunction
with NIDA Blending Conference in Cincinnati, OH. TOT held June 4-6, 2008 with 22 participants. Total of 64 trainees from May 2007- June 2008.
II. Mid-Atlantic ATTC sponsored four trainers to be trained in PAMI. Two at the PAMI TOT after the Blending Conference( Susan Pauley- VA and Michael Gosser- KY) and two at the Tennessee Advanced School( Adam Webster- Chattanooga, TN) and Barry Cooper-Jackson, TN). Both Ms. Pauley and Mr. Gosser were sponsored to attend the conference prior to the TOT.
III. Mid-Atlantic ATTC sponsored two trainers to attend the MIA:STEP in conjunction with the NIDA Blending Conference. The trainees are Karen Garrity – Kentucky and Rebecca Stacey- West Virginia. In addition, two other Mid-Atlantic ATTC WV trainees were sponsored by their CTN node, giving us three trainers for WV.

c. The Mountain West region nominated two people to participate in the MIA-STEP TOT. One participant refused to submit a tape and the second participant’s tape did not meet the minimum criteria. Consequently, the Mountain West did not send anyone to the most recent MIA-STEP TOT.

NIDA requested comprehensive maps showing all official blending trainings held by the ATTC Network since January 2005 in preparation for NIDA day at NASADAD annual meeting.
To ensure accuracy of the maps, the National Office requested that ATTC Regional Centers review their past event profile forms and, using our new understanding of how to categorize events, re-enter in the new events database all official blending trainings held since January 2005. The National Office then used the database to generate the maps and sent the maps to the ATTC directors for two separate rounds of review.

(4) Development of Ancillary Products The ATTC National Office coordinated the redesign, reproduction, bulk purchase, and
distribution of a PAMI Blending Initiative product on behalf of the ATTC Network. The National Office ordered 3400 PAMI folders at $4/piece. The National Office delivered the products to Regional Centers in May 2008.
The ATTC National Office coordinated the reproduction, bulk purchase, and distribution of nearly 1800 of the NIDA Blending CD Kits, originally produced by NIDA. The Blending CD kit, which costs approximately $15/piece, consists of five CDs and inserts that contain presentations on each of the five blending products. The CD was delivered to Regional Centers, CSAT and NIDA during May 2008. The National Office also began a second round of printing for this product in June. It is anticipated that 1,000 more CD sets will be produced in the next quarter.
The ATTC National Office is chairing a sub-committee of the Network’s NIDA Blending workgroup. The sub-committee will update and enhance the web pages on the Blending products for the new ATTC Web site.

. Since last quarter the Northeast ATTC has worked with key stakeholders in the region
to disseminate the five NIDA Blending products through the Single State Authorities. Dissemination activities that took place this quarter consisted of a mailer of the Blending CD-ROM kit; presentations of Blending products to the Pennsylvania Drug and Alcohol Coalition meetings; emailing list serve announcements of the Five Blending Products and offering on-site NIDA Blending Product training in the specific curricula. NIDA Onsite trainings – The Northeast ATTC is working with six treatment providers and the New York State Addiction Treatment Centers (13 sites) to do onsite NIDA Blending trainings. The providers that have been identified for the on-site trainings are Palladia, Inc and the Palladia Starhill Facility, NY, Southwest Behavioral Health Management, Inc., PA, RC-MTP, Pomona, NY, Beth Israel Medical Center, NYC and New York Training Institute, Phoenix House, NY.
Since the last quarter, Dr. Flaherty has conducted bi-weekly teleconferences and two face-to-face meetings with the Pennsylvania Department of Health, Bureau of Drug and Alcohol Programs and PA Department of Public Welfare, Office of Mental Health and Substance Abuse Services and other stakeholders (Pennsylvania Drug and Alcohol Coalition) to create the Pennsylvania Drug and Alcohol Coalition. The goal of this coalition is to identify and build a coordinated system of care in Pennsylvania

capable of addressing in a collaborative manner the needs and priorities of Pennsylvanians for coordinated healthcare in the prevention, intervention, treatment and recovery of substance use and co-occurring disorders. The Pennsylvania Drug & Alcohol Coalition meetings were conducted via conference calls (April, 18, 2008; May 9, 2008; May 23, 2008; June 9, 2008 June 20, 2008; and face-to-face meetings (May 12, 2008 and June 30, 2008).

The Northeast ATTC is planning to conduct a workshop on an overview of evidence-based practices. Approximately 60 participants are expected. The Northeast ATTC is in planning and development of the following trainings for Oct. 2008 - Dec 2008: Northeast ATTC Plan for 7/1/08 - 9/30/08. The Association of Addiction Providers of New York (AAPNY) conference – October 12 – 14, Saratoga, NY. The Northeast ATTC will conduct two of the 6 hour NIDA Blending Trainings @ 20 participants per training and one PAMI workshop @ 20 participants per training. On June 2, 2008 in Cincinnati, Ohio, Thomas Freese, Ph.D. (together with Mary Ann Detmer, LPN, and Gregory Brigham, Ph.D.) presented “Understanding and Utilizing Buprenorphine in the Treatment and Short Term Detox of Opioid Addiction” at the NIDA Blending Conference: Blending Science and Treatment – The Impact of Evidence-Based Practices on Individuals, Families, and Communities. Tom provided a one-hour overview of the two approved buprenorphine-related BI packages, and Greg and Mary Ann discussed their experience with participating in the CTN buprenorphine trials and the challenges of incorporating the BI packages into a real-world treatment setting.
III. Christie Thomas (Pacific Region Node), Dr. Michael Shopshire (CA/AZ Node), and Dr. Andy Andersen (ASU CABHP) represented the PSATTC in the PAMI Training of Trainers session held in conjunction with the NIDA Blending Conference in Cincinnati, Ohio on June 2-3, 2008. Ms. Thomas and Drs. Shopshire and Andersen will conduct PAMI training sessions throughout the region on an as-needed/ requested basis.
c. Other I. Beth Rutkowski volunteered to participate in a small workgroup to update the
Blending Initiative section of the new National ATTC website. The workgroup had its first meeting via conference call on May 29, 2008, and had a subsequent conference call on June 3rd and 19th. Other members of the workgroup include Laurie Krom (NATTC) and Jennifer Norland (MWATTC).

The PSATTC, together with the Center for Health Care Evaluation, Program Evaluation and Resource Center within the VA Palo Alto Health Care System and Stanford University School of Medicine (Ken Weingardt) is co-sponsoring an online/correspondence course entitled, “NIDA Clinical Toolbox.” The four-hour, 8-module course is designed to provide practicing substance abuse counselors with

the knowledge, skills, and abilities required to successfully implement Cognitive Behavioral Therapy strategies and techniques in their clinical practice (based on a NIDA grant). The course officially launched in June 2008, and was featured in the July 2008 issue of Eye on the Field (and is posted to Addictioned.org). In just a three day period from July 7-9, more than 160 people accessed the online course. UCLA ISAP and the National ATTC Center are providing CAADAC and NAADAC continuing education credit, respectively.

(2) Challenges and Strategies Takena. Infrastructure development and multiple communications continue to be necessary
with Single State Authorities (SSA) Directors, state program associations, and program directors to prepare for the dissemination of Blending Products in the PATTC region. Dr. Anne Helene Skinstad and Candace Peters, MA, CADC, consistently communicate with these agencies and organizations to prepare for implementation of Blending Products.
b. The States of Iowa (IA) and Wisconsin (WI) have experienced extreme weather conditions the whole year of 2008. Since April, these two states have had an extreme flooding situation which has seriously inhibited communication. The home office at the University of Iowa had to be evacuated and no staff members were allowed into the office for almost two weeks. Furthermore, communication through e-mail is still affected and undependable.
c. Infrastructure assessment and thorough needs assessment of the workforce continues to be necessary in the PATTC region before Blending Products can be disseminated. This is especially true and necessary with Motivational Interviewing prior to the roll out of the MIA:STEP Blending product. Three of our states have had limited to no training in Motivational Interviewing still, and it has been difficult to nominate clinicians proficient enough for the MIA-STEP TOT. Several of our nominees have not been accepted into the TOT. At this point we have one trainer Mark Dodd, MSW, from IA and Dr. Kate Speck from Nebraska (previously part of the PATTC) who can train the MIA:STEP Blending Program.
d. Opioid medication treatment is not readily utilized in the entire PATTC region and requires extensive pre-training preparation with multiple stakeholder relationships prior to implementation. However, the need for treatment on the use of Buprenorphine with opiate addiction is increasing, because of the increase in abuse of pain-killers in the states of South Dakota and North Dakota. The PATTC region continues to have difficulty disseminating the Buprenorphine and Buprenorphine Detox Blending products as released. Buprenorphine is being disseminated adaptively in 60-90 minute sessions throughout the region; however the length of the current product is prohibitive.

collaborate with Dr. Eugene Somoza, PI for the Ohio Valley CTN node on including substance abuse community treatment programs (CTPs) within the Aberdeen Area Indian Health Services in clinical trials. Dr. Mackey has met with Dr. Eugene Somoza and staff from the Aberdeen Area Tribal Chairman’s’ Health Board to prepare to include tribal providers in upcoming clinical trials.
c. Dr. Skinstad met with Dr. Mackey and Dr. Somoza to discuss further collaborations and also how to facilitate PAMI training with providers in the Ohio CTN Node.

e. PATTC continues to collaborate with Professor Ken Winters at the University of Minnesota, Department of Psychiatry.

PATTC continues collaborations with Professor Paul Moberg, University of Wisconsin School of Medicine and Public Health, Department of Population Health Sciences and Deputy Director, UW Population Health Institute. The goal of our collaboration is to plan the dissemination of results from the NIDA funded project on Recovery High Schools.
g. Dr. Skinstad continues to collaborate with Dr. Rob Philibert at the University of Iowa, Department of Psychiatry, Genetics Laboratory. This lab is a long standing laboratory funded by NIDA. Dr. Skinstad, Dr. Rob Philibert, and Dr. Don Black are preparing a NIDA grant submission.

Georgia Recently, Southeast ATTC met with the Executive Director of the National Opiate Treatment Providers Association who advised, in conjunction with the cost issues, that it has been quite challenging for her doctors to switch to Bup because they have used Methadone for so long and the clients they serve are not as prone to change when the methadone has been working for them. She confirmed that cost has been the primary reason because once the patients begin using BUP they are not able to afford refills.
SATTC GA will be initiating a follow-up meeting with the President of NOTCA because the ED has since resigned her post. During this meeting SATTC and NOTCA will discuss ways to collaborate on ways to disseminate the BUP information to their target population. SATTC GA will also be exhibiting and disseminating BUP information at the International Nurses Society on Addictions October 1-4 in Atlanta, GA. This conference will be an educational venue to introduce nurses throughout the region on BUP.SATTC GA is in the process of planning an informative session on BUP, inviting physicians, hospital staff, and pharmaceutics. This session will be in collaboration with the National Center for Primary Care at MSM. An invitation will also be extended for the President of NOTCA to participate on our Advisory Board.
Strategies will be formulated and implemented throughout the various collaborative entities on positive ways to disseminate and familiarize individuals with the BUP NIDA product.
