FLWG “Orders”

• “Orders” are our authorized pilot lists.• FLWG is using lists generated by the NHQ

MIMS/Operations Qualifications reports function in place of the current orders for some pilot operations.

• Pilots who have all their info in Ops Quals, get validated, and approved will find their names on the appropriate list the day after the process is complete and will not have to wait until the next months orders are issued, or for temporary authorization. It is therefore imperative pilots have their information in Ops Quals.

Stan/Eval at the unit level

• The process of getting a Form 5 for new members can be smoothed out by mentoring the new pilots. Get them into E-services and Flight Schedule Pro. Help them with getting their online exams and OPSEC tests done.

• Familiarize them with the requirements and paperwork needed. Show them how/where to download the forms.

Paperwork Changes

• The FLWG Form 12 “Pilot Data Summary” is now an optional form. Check pilots and units may use this if they choose. It is now un-necessary to send one up the chain.

• The check ride paperwork flow is changed.

The FLWG Form 11 is still required but no Form 12 needs to be with it.No copy of license and medical needed either.

Check rides• Insure the Form 5 or 91 is filled out, signed and

dated. It helps to put the applicants CAP ID and email address in the Comments box. Refer to handout.

• Enter the pilot and check ride info into the NHQ Operations Qualifications (The link is labeled “My Operations Qualifications/National Reports)

• The check pilot should be the person to validate that information. Commanders and Ops officers can also do this but it it highly desirable that the check pilot do it.

• Send a copy of the Form 5 or 91 to group DOV.

Modified paperwork flow

• After a successful ride, the Check Pilot works with the local Ops officer to get the pilot approved for such operations as cadet O-pilot, instructor, etc. After validation, this entry will show in Ops Quals as “Pending unit approval” until this is done. The FLWG Form 11 Request for pilot authorization and the associated copies of license, medical, etc. need to go onward. Copies of exams, questionnaires, etc to the wing are not needed.

Flow, con’t

• Check Pilot then sends the F5 or F91 to their Group Stan/Eval officer – NOT direct to the wing. The Group Stan/Eval officer checks again for accuracy and then works with the Group Ops officer to approve the pilot function in Ops Quals. Prior to this approval, Ops Quals will show “Pending group approval” in the pilot’s information.

Wing approval

• The Group Stan/Eval officer now sends the Form 5, Form 5G, or Form 91 to the Wing. The e-mail address is [email protected]. This will automatically send copies to the DOV and wing Ops Admin. Check the Stan/Eval page for a mailing address if desired.

• A copy to the wing of any check ride form is mandatory. It is used for trend analysis. Make sure it is complete, signed, dated and submitted in a timely manner.

Having a problem?

• Insure the box(es) indicating minimum PIC time is checked.

• Insure your exam(s) is in there• Insure your medical and BFR dates are

entered and current. You will show as expired when either lapses.

• Sometimes entering a basic C-172 or C-182 check ride helps.

Dead Ends

• The Wing DOV can’t help you if the unit and group officers have not validated and/or approved you.

• The Wing DOV can’t issue any authorizations without seeing the complete information in Ops Quals.

• Get approved at those levels before sending requests. This will expedite the process.

National Reports

• Any member may access these lists (“orders”) in e-services, on the left side under My Operations Qualifications/National Reports. It is the same area you entered your pilot data in.

• Click National Reports, select the unit number and the desired report.

FAQs, Tips and Advice

• Can I sign off the instrument portion if I am not a CFII?

• Know that the Form 5 and 91 are legal documents.

• Use “Rational Flexibility”.• Write in the t/o and lndg distances on the

F5.• The comments box…

When can I start?

• Get your Form 11 signed and sent ASAP to be approved as a check pilot.

• You will get validating authority thru the DO or DOV after that is accomplished.

• When approved in Ops Quals and on orders you can start conducting rides.

• See you back here in 4 years. (Used to be three but….)

• Check that applicants have OPSEC accomplished

• Insure mission pilot applicants have 101 and radio cards

• Do they know how to handle a mishap, what a Form 78 is?

• Be conservative and use DIMS – Does It Make Sense?
