As a generator and/or preparer of sewage sludge, NYCDEP has provided the following requisite information, in each annualreport, for the reporting period of January 1, throughDecember31, 2007: d. Name and address of the preparer and/orland applier who receivedthe sewage sludge next; Analytical results, reported as milligrams per kilogram, expressed as a dry weight, frommonitoring pollutant concentrations in the sewage sludge. Also included, are the prescribed analytical methods, the frequency of sewage sludge sampling/monitoring, and the types of samples collected; g. A certification statement and description of how the pathogen reduction requirements were met, including, thetype of process used, thestandard operating procedures;

Pursuant to Title 40 Code of Federal Regulations, Part 503 Federal Standards for theUse or Disposal of Sewage Sludge (40 CFR Part 503), the City of New York,Department of Environmental Protection (NYCDEP) has submitted herewith, oneannual report for each of the fourteen (14) Publicly Owned Treatment Works(POTWs) operated under the direction ofNYC DEP.

h. A description of how the vector attraction reduction requirements were metand a certification statement that the vector attraction reductionrequirements were met; and,

1. A listing of all relevant environmental (Federal, State or Local) permits orconstruction approvals received or applied for.
