OBJECTIVE To implement correction of student’s name, date or place of birth. 2. SCOPE This is applicable to students whose name, date or place of birth reflected in their records do not tally with those in the birth certificate. 3. REFERENCES 3.1 CHED-NCR Memorandum No. 3, s. 2000 3.2 Manual of Regulations for Private Higher Education (MORPHE) 4. POLICIES 4.1 The student’s name, date and place of birth as indicated in the birth certificate or alien certificate of registration shall be reflected in all his/her records. 4.2 Requests for correction of name, date and place of birth shall be acted upon by the University Registrar as delegated by the Commission on Higher Education (CHED) 4.3 The University Registrar/Registrar shall act on requests for correction provided the correction needed does not require court action. 4.4 When requesting for correction of name, the applicant shall attach the following: 4.4.1 Letter of Request for Correction of name 4.4.2 PSA Birth Certificate 4.4.3 Student’s/Parent’s Affidavit 4.4.4 Joint Affidavit of Two (2) Disinterested Persons Note: Three (3) copies each (1 original and two duplicate/xerox copies) The use, disclosure, reproduction, modification, transfer of the document for any purpose in any form or any means without approval from CEU is strictly prohibited, and may be subject to disciplinary and/or legal action.

CORRECTION OF NAME, DATE OR PLACE OF BIRTH 1. OBJECTIVE

To implement correction of student’s name, date or place of birth.

2. SCOPE

This is applicable to students whose name, date or place of birth reflected in their records do not tally with those in the birth certificate.

3. Verifies records/documents and forwards to the SRA

4. Checks entries on forms. If documents/records are in order, returns to SRA In-charge (Encoder).

5. Sends link for Online Payment

8. Signs the form

9. Informs the client if the document is done.
