Aesthetic Component O/No 8000-944

IOTN - Index Of Orthodontic Treatment NeedThe Index of Orthodontic Treatment Need (IOTN) assists in the ranking of malocclusion in terms of the significance of various occlusal traits for an individual’s dental health and perceived aesthetic impairment, with the intention of identifying those individuals who would most likely benefit from orthodontic treatment. The index incorporates an Aesthetic and Dental Health Component.

Aesthetic ComponentThe Aesthetic Component consists of a scale of ten colour photographs showing different levels of dental attractiveness. The dental attractiveness of prospective patients can be rated with reference to this scale. Grade 1 represents the most and Grade 10 the least attractive arrangements of teeth. The score reflects the aesthetic impairment. Photographs are used for dental cast assessment.

The Dental Health Component The Dental Health Component facilitates the synthesis of the current evidence for the deleterious effects of malocclusion. The DHC records the various occlusal traits of a malocclusion which would reduce the longevity of the dentition and surrounding structures. There are five grades, Grade 1 and 2 representing no need for orthodontic treatment, Grade 3 a borderline need for orthodontic treatment, and Grades 4 and 5 requiring orthodontic treatment on dental health grounds. Only the worst occlusal trait is recorded.

The Dental Health Component Ruler (DHC RULER)This ruler has been designed to provide the information necessary to record the DHC. The ruler has been developed for the clinical setting in which information is collected regarding competence of the lips, discrepancy between retruded contact position and intercuspal position on closure and masticatory/speech problems. Both disposable and autoclavable versions now available.

The PAR Index (Peer Assessment Rating Index)The Peer Assessment Rating Index was developed as a result of the report of the Occlusal Index Committee, which was itself established in 1986 as a direct result of the recommendations of the Schanshieff Report.

The PAR index was constructed initially with the help of the British Orthodontic Standards Working Party and was subsequently validated by weighting the constituent components to conform with the collective opinions of 74 clinicians (specialists, consultants, GDPs, community dentists).

The PAR Index measures the degree of departure which the occlusion of a set of study models has from the ideal. Hence it can be used to measure treatment change by calculating the difference between the scores obtained from pre- and post treatment models.

The index is the summation of several different components which make up malocclusion. Crowding or spacing is assessed in each arch in terms of contact point displacements. Labial occlusion is scored similarly in terms of antero-posterior, transverse and vertical discrepancy (overbite, overjet, centrelines).

The weighted version of the PAR Index results from the opinions of the 74 clinicians. There are only 7 components which are scored: labial irregularity (upper and lower), buccal occlusion (left and right), overjet, overbite, centreline.

The PAR RulerThe PAR Ruler has been designed to make measurement easier. The information briefly summarises the recording features of the index and facilitates quick assessments in relation to the seven components.

The PAR Ruler is now available in an autoclavable version.
