1 19 JUNE 2015 Pass it on… ADISORY: HAIL DAMAGE CLAIM SUPPLEMENTAL GUIDANCE This transmittal provides supplemental guidance to the email published on June 5 and explains how to file a claim with the Military Claims Office (MCO). Claimants are required to file with their insurance company prior to filing with the MCO. After you settle with your insurance company, you may submit your claim for the entire cost of repair to the Fort Carson Military Claims Office. The MCO will adjudicate your claim under the Personnel Claims Act (PCA) to determine if any amount in addition to your insurance settlement may be offered. If the cost of repair is less than the amount of your insurance deductible, you may file directly with the MCO. In that case, your claim must include proof of your deductible cost as well as an estimate or invoice for repair. Likewise, if your insurance does not provide coverage for this type of loss, you must provide a copy of the policy with your claim. To submit your claim to the MCO, you must log into the Personnel Claims Army Information Management System (PCLAIMS). PCLAIMS can be accessed at the Judge Advocate General's Internet site. Enter www.jagcnet5.army.mil/pclaims in your browser's address line. Click the Claims link on the right side and then click on the PCLAIMS link on the left side. The PCLAIMS link will describe the rules for filing personnel claims and allow you to fill out all of the required forms. You must use an Army Knowledge Online (AKO) account, or CAC card, to use PCLAIMS. Claims submitted on PCLAIMS should have the following documents uploaded: 1. A completed VID number (Vender ID #) Request form (open attachment at link provided below). This form needs to be returned to this office by email at [email protected] so that we can electronically request a VID# from DFAS to process your claim for payment. VID numbers may take approximately seven days to be issued.

2. Proof of motor vehicle registration and insurance 3. Proof you filed with your insurance company 4. One estimate of repair 5. Military police report 6. Pictures of the damages, including a picture of the entire car 7. Power of attorney to file claim (if filed by other than the claimant/owner.) If filed by a spouse, a written authorization by the sponsor will suffice. 8. A signed and completely filled in DD Form 1842. 9. E.A.C.H. Dining facility Hot meals on the weekends will soon be back at the Evans hospital Dining Facility. The Nutrition Care Division (NCD) understands the significance of providing hot meals to our patients' guests and to our staff members on the weekends. In response to an outpouring of requests, on July 4, NCD will restart this offering. The Dining Facility's weekend hours are Breakfast: 6:30 -- 8:30 a.m.; Lunch: 11 a.m. -- 1 p.m.; Dinner: 4 -- 5:30 p.m. This will have no impact on our meal service to inpatients. FEDS FEED FAMILIES CAMPAIGN The Office of Personnel Management is encouraging all Department of Defense staff to contribute to a local food bank this summer during the Feds Feed Families campaign. Army Community Service Community Connections Building 1524 will offer a drop off site for all nonperishable foods now through Aug. 21. Food dropped off at the Community Connections building will be donated to a local food bank on behalf of all Fort Carson federal employees. Donations benefit the local community. FORT CARSON PEER FORUM (FLYER) The Fort Carson PEER Forum will be held this month, 25 June from 1400-1600 @ the SFAC. All Military Caregivers, regardless of status, are allowed to attend – this includes IDES/MEB SM Caregivers, Veteran Caregivers, remotely located Caregivers, etc.

FREE SMUCKER’S BREAKFAST A free Mission Breakfast, a J.M. Smucker Company Breakfast Event, takes place 7:30-10 a.m. June 24 at the Special Events Center for the Fort Carson community. Patrons will be directed to park in the dirt parking lot north of the SEC, grass lot south of the SEC, across Wetzel Avenue at Iron Horse Park, the Fort Carson Commissary and other surrounding parking lots. Handicap parking will be located in the SEC's east parking lot off of Specker Avenue. Patrons are encourage to car pool.

SCHOLARSHIP ESSAY CONTEST The Sergeant Audie Murphy Association Mountain Post Chapter is running a $2,500 college scholarship essay contest. The deadline to enter is June 25. Children of active-duty, enlisted Army Soldiers graduating in 2015 are eligible to enter. The topic for the essay is “My Life as a Military Child.” The essay must be 1,500 words and double spaced. The winner will be selected by a pane of Sergeant Audie Murphy Association members. Entrants should submit the essay, a college acceptance letter and the sponsor’s assigned unit information to [email protected]. Include contact information with email – full name and phone number of entrant and sponsor. TBI STUDY The Defense and Veterans Brain Injury Center (DVBIC) research team on Fort Carson is actively recruiting for a new TBI non-drug treatment study. The purpose of the study is to see if adding a new type of brain training can help people who have had a concussion with symptoms such as memory, concentration, and emotional regulation. Study participation involves an intensive program of up to 20 treatments over six weeks and is non-invasive with minimal risks expected. We are looking for TRICARE eligible service members and veterans between the ages of 18 and 50 who have had a mild to moderate TBI within the past five years, and are still experiencing symptoms.
