1 TO: PROSPECTIVE BIDDERS DATE: OCTOBER 23, 2013 RE: ADDENDUM #1 - BID NO. 2013000426 BURNT STORE ROAD PHASE III CONSTRUCTION BID OPENING DATE: 2:00 p.m. (EST), NOVEMBER 6, 2013 Bidders are hereby notified that this Addendum shall be made a part of the documents. The intent is to add to, modify, or clarify the documents. Should any of these items have an effect on price, such changes shall be included in the price bid. These items have the same force and effect as if contained in the original. ITEM 1: QUESTIONS/ANSWERS Q1. While visiting the job site, we noticed powerline posts and line are along the existing road and most probably will be in the middle of the new road. Will these powerline posts and lines be relocated before project starts? A1. See Special Provision SP-11Cooperation with Utilities. FPL is aware of the project and has been compensated for conflicts. Q2. For the Restrained Bell and Spigot (B&S) pipe sections that require casing, 36” steel casing with spacers is specified for the 24” B&S pipe, 32” steel casing with spacers is specified for the 20” B&S pipe, and 28” steel casing with spacers is specified for the 16” B&S pipe. Can Fusible PVC™ be used as an equal to B&S PVC for the steel cased sections of the project with downsized steel casings? Can Fusible PVC™ be used as an equal to HDPE for the Directional Boring sections of the project? A2. After reviewing the request by Underground Solutions, Inc., Charlotte County Utilities (CCU) has denied the use of Fusible PVC as requested. Q3. Can the bid tabulation be provided in an excel file to reduce typing? A3. No. Q4. Has the entire ROW for this project been acquired? A4. Yes. Q5. Item U-2 - Fittings (8" & up) is showing TN. Is this tons? A5. This item is to be changed to lbs. ITEM 1: QUESTIONS/ANSWERSQ1. While visiting the job site, we noticed powerline posts and line are along the existing road and most probably will be in the middle of the new road. Q7. Item 400-4-1A - Concrete Class IV, Traffic Barrier, 32" F Type (Coping, Ret. Wall, Junct. Slab, Sidewalk) has 5,902 CY. On Sheet 283 of the plans there is a detail of a wall but the quantities are less. Where can I find the other quantities?A7. The correct quantity is 59.2 CY and has been reflected on revised Bid Form page 50, which can be obtained by accessing www.charlottecountyfl.com/purchasing under “Purchasing Bids Online”, document #134266a.

Q8. Sheet 3 calls for the temporary guardrail End Assemblies to be part of pay item 102-73, but there is not a pay item 102-73. Please clarify how the temporary guardrail pay items will be handled. A8. Temporary guard rail (FDOT pay item 102-73) is to be paid under pay item 102-1 Maintenance of Traffic.

Q9. Where is the relocation schedule for the overhead electric? A.9 See page 20, SP-11 Cooperation with Utilities.

ITEM 2 REVISIONS/CLARIFICATIONS1) At STA 166+80 rt side, there was a CCU 24” water main conflict with an existing sign (Allison James Realty Sign). The water main was shifted west. This change affected five (5) plans: Sheet Nos. 13, 14, 33, 47 and 48. Revised plans can be obtained by accessing www.charlottecountyfl.com/purchasing under “Purchasing Bids Online”, document #134266a.

Bidders are required to acknowledge receipt of this addendum on their bid forms. All other terms and conditions of the originalbidding and contract documents remain the same.
