DEAR COLLEAGUES,St. Louis Children’s Hospital is proud to present our 12th Annual Perspectives in Pediatrics Conference on April 26, 2018. As a top-ranked children’s hospital, we continually strive to improve pediatric health care locally and across the country. In keeping with this goal, this year’s conference will focus on the theme of “Innovative Practices in
Pediatric Healthcare,” highlighting work being done by a variety of our inter-professional teams that is breaking new ground and improving care for children and youth.
Topics planned include an innovative approach to health care screening of adolescents in the emergency department, palliative care for complex chronic conditions and innovations in pain management. These are important topics in light of our current opioid epidemic. Effective approaches for assessing risky behavior in adolescents and managing pain in young people are strategies that can prevent future opioid dependency in adulthood.
We recently launched a specialty clinic for children with gender dysphoria and disorders of sexual development. An expert physician on this topic will share what we are doing as the only provider of this service in the region.
In addition to innovations in care for children, we will share our efforts to support our staff facing secondary trauma associated with situations at work. The We Care Program offers support from specially trained local peers in the aftermath of a critical incident. Many clinicians have found We Care extremely helpful.
Lastly, several of our team members will be presenting the results of evidence-based practice projects they have completed to improve patient care within the hospital.
If you are from another pediatric organization, you’ll hear about innovative practices that you can take back to your own organization. As pediatric health care providers, we need to share our new ideas, evidence and practices to provide ever more effective and value-based care. We hope that you will join us for an exciting day of learning and networking with your colleagues!

KEYNOTE SPEAKERRussell G. Hoffmann III, PhD
Russell Hoffmann is Vice President, BJC Institute of Learning and Development (BILD), for BJC HealthCare. He leads a team that provides system-wide services in the areas of Talent Management, Organizational Development, Learning and Career Development. BILD services support achieving BJC’s mission: To improve the health and well-being of the people and communities whom we serve.
Russell joined BJC 22 years ago, and previously served in leadership roles at the BJC-affiliated St. Louis Children’s Hospital. As the Director of Cultural Transformation and Organizational Effectiveness, he focused on aligning individual, team and organizational efforts with the hospital’s mission and vision. Russell also served as the Director of the Department of Psychology, where he led a clinical service in providing inpatient and outpatient care to youth facing medical and/or mental health conditions. He also served as a clinical psychologist during his tenure at St. Louis Children's Hospital, and an adjunct faculty member of the Department of Pediatrics, Washington University School of Medicine.
Russell received his graduate training (Master’s Degree and PhD) in clinical psychology at the University of Florida with a focus in child and family systems psychology.

An overview of St. Louis Children's Hospital's Pediatric Advanced Care Team (PACT), also known as palliative care. At its core, palliative care is a comprehensive approach to care that offers an extra layer of support from an interdisciplinary team of doctors, nurses, social workers, chaplains and other pediatric specialists. Such care provides relief from the symptoms, pain, psycho-
social, emotional and spiritual stress of disease for both the patient and the family. It is appropriate for any stage of illness and can be concurrent with curative treatments. A panel discussion will follow to answer audience questions.

Spotlight presentations by St. Louis Children’s Hospital Research Fellowship Participants
A Randomized Controlled Trial Comparing Reduced Flat Times versus Standard of Care in Pediatric Patients Post Cardiac Catheterization
Pediatric patients undergoing cardiac catheterization procedures are required to lie flat for four to six hours and at times up to eight hours. Results of research in adult patients were found that flat time could be safely decreased to as little as one-and-a-half to two hours post cardiac catheterization procedure. The purpose of this study was to determine if decreased flat time for the post cardiac catheterization pediatric patient would affect the incidence of site bleeding, additional sedation and need for a critical care admission.

Pain Control and Positioning in Children Following Selective Dorsal Rhizotomy Surgery
A practice was changed from elevating the head of bed (HOB) on day one to elevating HOB on day three following a Selective Dorsal Rhizotomy surgery with the goal to improve pain control. The purpose of this mixed methods study was to determine if better pain management was achieved with this practice change. Both quantitative and qualitative results supported a return to one day of flat time.

“We’re so CRAFFT-y: The implementation of an alcohol and drug screening tool”
The CRAFFT screening tool is used to screen adolescents for alcohol and drug use. Previously, this screening was only done on admitted injured patients. In 2017, the Trauma Services team joined with Social Work and the patient care team on the 10th floor to extend this screening to all admitted patients age 12 and older. Simulation and roundtable discussions were used in the education plan prior to implementation.

This session will focus on pediatric multimodal pain management and the many factors that are driving changes. Reduction of opioid use, improved pain education and use of regional blocks for pain are some topics that will be explored.

Secondary Traumatic Stress in Health Care: Understanding the Impact on Caregivers
Secondary traumatic stress (STS) is the emotional and physical distress that health care staff can experience as they provide care. They are exposed to repeated loss, pain and suffering. The stress symptoms associated with STS can have a significant and lasting impact on the caregiver’s personal and professional life. This session will describe the trajectory of secondary traumatic stress and identify key strategies to support staff.

Caring for Transgender Patients
This lecture will help professionals who provide care and support for gender variant children to understand the health and social support needs of this population. Participants will learn current best practices for medical care of gender non-conforming patients. The knowledge gained by participants in this program will prepare them to improve the care they provide to transgender and gender-questioning youth and their families.
