Instructions for Continuing Education Credit

The Medicare Learning Network® (MLN) recently upgraded its Learning Management and Product Ordering System (LM/POS).

If you were already registered in the former MLN Learning Management System (LMS), you do not need to create a new login or password for the LM/POS. However, the appearance of the system and instructions for registering, logging-in, accessing courses, and obtaining certificate information have all changed.

Learning Management System (LMS) Instructions In order to receive continuing education credits (CEU) for a conference session, you must pass the post-assessment and complete the evaluation. The continuing education post-assessments and evaluations are being administered through the Medicare Learning Network®.

Post-assessments are located on the Medicare Learning Network® (MLN) Learning Management and Product Ordering System (LM/POS). Participants will need to login or register to access the post-assessments.

Registering To Take a Post-Assessment If you have previously taken Medicare Learning Network® (MLN) web-based training (WBT)courses, you may use the login ID and password you created for those courses. If you are a new user, you will need to register.

Supercharging PDSA: How to Deliver Real Change That Sticks (Activity #: IP-12142016-1)

Activity DescriptionAcross the health care industry, quality and improvement specialists know how hard it is to make change that is both effective (more efficient and provides better care) and fully adopted by stakeholders. How might we optimize change processes to really ‘stick’ and, better still, create a platform for catalyzing patient and provider engagement? How might we design new systems and processes that make it easy for patients and providers to adopt the behavior changes we seek? These are challenges rooted in the human experiences of systems.

The most effective approaches to health service and system innovation have harnessed the proven tools of design, combining qualitative ethnographic research and rapid iterative experience prototyping to deliver change that is not only effective but also welcomed by stakeholders.

This master class will explore case studies demonstrating these methods and tools, include interactive exercises to practice these techniques, and look at how they apply to the improvement of health care systems.

She also has experience working as a Medical Device Engineer for GE Medical Systems, Inc. and Abbott Vascular. Robert Lister, presenter, has nearly 20 years of experience of innovating products, services, and care models, utilizing the tools of Design Thinking to create new solutions in the health care sector. He created solutions that delivered better care and outcomes, while respecting the complex networks of stakeholders in the health care environment. Mr. Lister has experience working with some of the world’s leading hospitals on models of care. He also worked with pharmaceutical and medical device companies to create new care experiences.

Mr. Lister joined world-leading innovation consultancy IDEO.ORG (IDEO) as a Mechanical Engineer, in 2001. He also became a Designer, specializing in drug delivery and medical devices in 2001. As a designer, Mr. Lister established and lead IDEO's Global Medical Products Design Group. Mr. Lister subsequently joined AT&T as Director of Design to help establish their new international innovation center, The Foundry, in 2011. At The Foundry, he established a new team across three centers, which led to Mr. Lister becoming a Co-Founder of Future Medical Systems, LLC, a consultancy providing experience design services for the health care sectorin 2013.

Mr. Lister mentors teams for the Institute for Design (d.School) and BioDesign Fellowship at Stanford University (Stanford) and co-leads The Design Farm, a non profit educational initiative to teach design thinking to teens. In addition, he served as a Visiting Scholar for the Clinical Excellence Research Center (CERC) Design Fellowship at Stanford in 2016.

Karen Matsuoka, PhD, MA, presenter, serves as the Chief Quality Officer for Medicaid and the Children's Health Insurance Program (CHIP) for the Centers for Medicare & Medicaid Services (CMS) and directs efforts to assist States in providing a safe, efficient, and effective patient-centered care for program beneficiaries, other uninsured and vulnerable populations, and the overall National health care system.

Dr. Matsuoka brings multi-sector and interdisciplinary expertise to this work and has held positions in the Maryland Department of Health and Mental Hygiene, the Brookings Institution, the White House Office of Management and Budget, and the United States Congress' Ways and Means Health Subcommittee. At Stanford University (Stanford), Dr. Matsuoka led and facilitated several design workshops and engagements with organizations and providers that serve Medicaid beneficiaries. She was recently invited back to Stanford as a Visiting Fellow in the Institute for Design (d.School) and Clinical Excellence Research Center (CERC), applying human-centered design approaches to health system redesign issues.

Dr. Matsuoka earned her Doctor of Philosophy degree in Social Policy, as a Rhodes Scholar, at Oxford University. She also earned a Master of Arts degree in Philosophy, with an emphasis on Comparative Social Policy, at Stanford University.

Andrey Ostrovsky, MD, presenter, is a Social Entrepreneur and practicing Physician. He serves as the Chief Medical Officer for the Center for Medicaid and CHIP Services (CMCS). Prior to working for Medicaid, he co-founded and sold his software company, Care at Hand, an award-winning predictive analytics platform that used insights of non-medical staff to prevent aging people from being hospitalized. Under his leadership, Care at Hand published several peer-reviewed studies, enabled home and community-based service providers to sustain their care transition programs, and saved Medicare and Medicaid millions of dollars by preventing hospitalizations.

Dr. Ostrovsky has served on several boards and committees dedicated to interoperability standards, quality improvement and measurement, and innovation including the National Quality Forum, Office of the National Coordinator, and the Commonwealth Fund. Prior to Care at Hand, he led teams at the World Health Organization (WHO), United States Senate, and San Francisco Health Department toward health system strengthening through technology.

Dr. Ostrovsky holds a Doctor of Medicine degree and undergraduate degrees in Chemistry and Psychology, Magna cum Laude, from Boston University. He completed his pediatrics residency training in the Boston Combined Residency Program at Boston Medical Center and Boston Children's Hospital, where he was a Clinical Instructor at Harvard Medical School. Ms. Richter has more than 10 years of experience as an Experience Designer, innovating products, services, and care models and applying the tools of design thinking to the most complex and regulated markets. She has created solutions that delivered better care and outcomes, while respecting the complex networks of stakeholders in the health care environment and working with pharmaceutical companies, medical device companies, and some of the world’s leading hospitals on models of care.

Improving Antibiotic and Opioid Use: Sharing Experiences and Opportunities

Activity DescriptionThis master class will explore the current problems of antibiotic and opioid overuse. Presenters will outline some of the key barriers to improving prescribing of these agents and will emphasize some of the commonalities. Presenters will also discuss some of the key current activities aimed at improving the use of these medications. The goal of this session will be to share experiences that might have cross-over or even synergistic potential.

Deborah Dowell, MD, MPH, presenter, has been employed by the Centers for Disease Control and Prevention (CDC) as a Team Lead and Senior Medical Advisor, since January 2014. Dr. Dowell is also an Internal Medicine Physician. She conducted research on the effects of clinical guidelines and policies on prescribing and served as the Lead Author of the "2016 CDC Guideline for Prescribing Opioids for Chronic Pain." She has developed education for clinical providers on opioid prescribing in various formats such as printed materials, web-based trainings, and slide presentations. In addition, Dr. Dowell has conducted workshops and webinars on opioid prescribing for pharmacologists, clinicians, and health care researchers and administrators.

Mary G. Greene, MD, MPH, MBA, presenter, is a Pediatrician and Fellow of the American Academy of Pediatrics. She serves as the Director of the Governance Management Group in the Centers for Medicare and Medicaid Services' (CMS') Center for Program Integrity (CPI) and leads CPI-wide strategic and operational planning. Dr. Greene's work includes tracking and managing program integrity vulnerabilities, developing Medicare and Medicaid program integrity policy, regulations, and external reports, developing action plans to reduce improper payments, and monitoring performance. She also leads the development of CMS' Opioid Strategy. Her leadership has forged consensus both inside and outside of the Center on program integrity strategies, operational efficiencies, and vulnerability mitigations. Prior to joining CMS, Dr. Greene led strategy and operational support projects to help stand up new programs, gain efficiencies, build collaborations and foster professional development.

Captain Lauri Hicks, DO, presenter, is a Medical Epidemiologist in the Division of Healthcare Quality Promotion at the Centers for Disease Control and Prevention's (CDC's) National Center for Emerging and Zoonotic Infectious Diseases (NCEZID) and serves as a Captain in the United States Public Health Service (USPHS). After completing her residencies, Dr. Hicks served a two year position as an Epidemic Intelligence Service Officer at the CDC and a Post-Doctoral Fellowship in infectious diseases at Brown University. After completing her studies, she returned to the CDC to lead respiratory outbreak response and the Legionnaires’ Disease Program. She leads research on antibiotic use and resistance trends and serves as the campaign spokesperson. Dr. Hicks became the Director for CDC’s Office of Antibiotic Stewardship in the Division of Healthcare Quality Promotion to improve integration of stewardship across the healthcare continuum in 2015.

Catching Everyone in America’s Safety Net: Collecting Data on Sexual Orientation and Gender Identity in Health Care Settings

(Activity #: IP-12142016-3)

Activity DescriptionThis master class will serve as an introduction to lesbian, gay, bisexual, and transgender (LGBT) terminology, health issues, and ways to improve health quality for the LGBT population through data collection of sexual orientation and gender identify (SOGI) information. The material will begin with covering the basic terminology for lesbian, gay, bisexual, and transgender people and the difference between sexual orientation and gender identity. Followed by, why it is important to ask people about their sexual orientation and gender identity in clinical settings, LGBT health needs and disparities, and why SOGI data should be collected.

Throughout the training participants will gain valuable information about the Mapping Medicare Disparities (MMD) Tool, and how SOGI data would enhance it. The presentation will then discuss different ways to collect SOGI data in the clinical setting and recommend different ways to start implementing sexual orientation and gender identity questions into work flows with and without electronic health records.

The content of this class is applicable for any medical provider or health system and may be of particular interest for those serving the elder population, since special consideration is provided for working with older adults who may also be uncomfortable discussing their sexual orientation and gender identity.

Samuel C. “Chris” Haffer, PhD, presenter, is currently serving as Senior Manager for the Director of the Data and Policy Analytics Group in the Centers for Medicare & Medicaid Services Office of Minority Health (CMS OMH). In this role, Dr. Haffer directs and manages operational activities that relate to identifying and eliminating health disparities in under served populations. He develops new programs and initiatives, evaluates existing programs, and serves as an expert and principal source of advice and guidance to the OMH Director on health disparities, data analysis, research, and evaluation.

Dr. Haffer's experience also includes serving as a Program Director and Senior Technical Advisor in the CMS Office of Information Products & Data Analytics, where he led the Medicare Claims Data Public Use File Program and created free claims-based public use files. He also created the Medicare Data Sharing Program that allows performance measurement organizations access to Medicare claims data for developing report cards for health care providers, from 2011 through 2013. Dr. Haffer also served as Director of the National Medicare Health Outcomes Survey Program from 1997 through 2011.

Dr. Haffer is an Internationally recognized expert in the measurement and reporting of geriatric health outcomes, who authored or co-authored nearly two dozen articles in prominent peer reviewed journals. His research has focused on racial, ethnic, gender, and socioeconomic disparities in health outcomes of the elderly. He is currently exploring the impact of sexual minority status on health in the Medicare population.

The team accomplished this by culling and spreading best practices of high performing organ procurement organizations and transplant centers. Confidence Gbarayor, Ph.D., MPH, is an Epidemiologist and Health Services Researcher with over 15 years of combined experience in this field. Dr. Gbarayor joined the Centers for Medicare & Medicaid Services (CMS) as a Health Insurance Specialist in 2010. In this role, she served as the Lead Data Analyst for the Medicare Part D Prescription Drug Program for six years. Quality Measurement in Specialty Care – Oncology as a Case Study (Activity #: IP-12142016-7)

Activity Description This master class uses oncology care as a case study to discuss quality measurement in specialty care. It outlines the challenges in quality measurement in specialty care. Speakers representing different stakeholders will discuss how these challenges shaped early measurement in cancer care, how we currently measure quality in cancer care, and how we eventually hope to be able to measure quality in cancer care.

Target Audience This activity is designed for all health care professionals.

Learning Objectives By the end of this session, the participant should be able to:

• Identify both challenges unique to quality measurement in specialty and oncology care • Identify challenges shared with quality measurement in all settings • Identify opportunities for improving quality measurement in specialty care

Ronald M. Kline, MD, FAAP, presenter, is a Medical Officer at the Center for Medicare and Medicaid Innovation (CMMI). He is part of the program teams directing the Oncology Care Model and the Medicare Care Choices Model, as well as a Centers for Medicare & Medicaid Services (CMS) representative to the Cancer Moonshot Initiative. As a Robert Wood Johnson Foundation (RWJF) 2013/2014 Health Policy Fellow, Dr. Kline focused on health policy in the Office of Senate Finance Committee Chair Ron Wyden and CMMI.

Dr. Kline has been a clinical Pediatric Hematologist-Oncologist for over 25 years. He served as the Pediatric Medical Director for Comprehensive Cancer Centers of Nevada immediately prior to accepting his RWJF fellowship. Dr. Kline continues to be clinically active as a Medical Officer, volunteering in the pediatric hematology-oncology clinics at Walter Reed Military Medical Center and Johns Hopkins Hospital. He also works as a Pediatric Hospitalist and Emergency Department Physician at the Greater Baltimore Medical Center.

Dr. Kline earned a Doctor of Medicine degree and a Bachelor of Arts degree in Biology at the University of California, Los Angeles.

Rebecca Metzger, presenter, is employed by Telligen, who provides program implementation and monitoring services through a sub-contract with RTI to the Centers for Medicare & Medicaid Services (CMS) in support of the Oncology Care Model. Ms. Metzger is currently providing support for CMS Quality Measures initiatives and population health management solutions. She has 10 years of leadership experience providing support to CMS in implementation of quality initiatives across many care settings and CMS demonstrations. Ms. Metzger's leadership experience includes implementing population health programs focused in improving the quality of care, while reducing costs for Medicare, commercial insurers, and Medicaid populations.

Ms. Metzger earned a Bachelor of Science degree in Psychology at Drake University.

Eduardo Ortiz, MD, MPH, presenter, is the Director of Quality, Measures and Guidelines at the American Society of Clinical Oncology (ASCO). He is board-certified in internal medicine and has expertise in clinical medicine, evidence-based medicine, quality measures and improvement, guideline development, systematic review methodology, informatics, outcomes research, and public health. Dr. Ortiz's previous experience also includes serving as the Medical Director and Senior Fellow at Atlas Research, a veteran-owned small business that provides consulting services for Federal agencies to assess and improve the quality of health care. He also served as the Director of Clinical Development and Informatics at ProVation Medical and Wolters Kluwer Health, were he led the development and implementation of evidence-based order sets in the inpatient, outpatient, and emergency department settings in more than 700 academic and private clinical care organizations.

Dr. Ortiz has been an author on more than 40 journal articles, book chapters and government reports. He has delivered over 90 presentations at Regional, National, and International meetings. He has held academic faculty appointments at Georgetown University, Johns Hopkins, Harvard, George Washington University, and the University of California, San Diego.

Dr. Ortiz earned a Master’s Degree in Public Health at University of Utah Health Sciences Center. He completed a fellowship in General Internal Medicine and Medical Informatics and Salt Lake City Veterans Administration Medical Center.

Tracy Spinks, presenter, has been employed by the University of Texas MD Anderson Cancer Center, a National Cancer Institute-designated (NCI-designated) comprehensive cancer center since 2010. Ms. Spinks currently serves as the Program Director for Cancer Care Delivery. She is responsible for the development of oncology focused health policy, cancer-specific outcomes, and alternative payment models. Ms. Spinks also serves as the Program Director for the Alliance of Dedicated Cancer Centers' Quality Committee, where she provides tactical direction for measure development. Her previous experience includes providing management and litigation consulting services to clients in oil and gas, manufacturing, and other industries.

Ms. Spinks has over six years of experience in cancer quality measurement. She is well versed in cancer quality measurement and measure development. She presents on cancer related topics

Andrew York, PharmD, JD, presenter, is a Pharmacist and Attorney in the Patient Care Models Group (PCMG) of the Center for Medicare and Medicaid Innovation (CMMI). Dr. York works on the program team for the Centers for Medicare & Medicaid Services (CMS) Oncology Care Model (OCM), where he led the development of the OCM Quality Strategy. He serves as a Subject Matter Expert (SME) on OCM clinical and quality measures. Prior to working at CMMI, Dr. York was an Oak Ridge Institute for Science and Education (ORISE) Health Policy Fellow at the Department of Health and Human Services (HHS) Office of Disease Prevention and Health Promotion (ODPHP), where he helped develop the HHS National Action for Adverse Drug Event Prevention.

Dr. York has developed education and training materials for quality measurement for the Oncology Care Model in various formats, including printed materials, webinars, and slide presentations. He has conducted webinars, office hours, and one-on-one training for OCM providers on quality data collection and submission.
