What is Self Management“Self care is what each person does on a daily basis. This
is often compromised for a person living their life with long term conditions.”
“Self management is the process each person develops to manage their conditions”
“Support for self care and self management is the responsibility of health and social care providers and paid carers.”
LTCAS summarised the 2 definitions as “Self management is the successful outcome of the person
and all appropriate individuals and services working together to support him or her to deal with the very real implications of living the rest of their life with one or more long term condition” (LTCAS, Guan Yersel, 2009)
