POST: CONSULTANT IN OLD AGE PSYCHIATRY (10 PAS)
BASE: DUMBARTON JOINT HOSPITAL
This is an exciting and well established Older Adults Service in West Dunbartonshire HSCP. Within West Dunbartonshire HSCP, there are 2 Older Adults services; Clydebank and Dumbarton. The post holder will be responsible for the service based in Dumbarton, with an approximate population of 11,000 older people.
The post holder will have Inpatient, Older Adults Liaison and Community responsibilities. The inpatient unit is based at the Vale of Leven Hospital and comprises a 12 bedded dementia assessment ward and a 6 bedded functional ward. The post holder will be responsible for Older Adults Psychiatric Liaison at the Vale of Leven Hospital however the majority of all assessments are undertaken by the Specialty Grade Psychiatrist. The post holder will be supported by a highly skilled and confident Older Adults Community Mental Health Team (OA-CMHT) and Care Home Liaison CPN who will undertake the majority of initial assessments. There is also a well established OA-CMHT duty system for emergencies and urgent assessments.
The appointee will be expected to be an Approved Medical Practitioner and to carry out work under the Mental Health Act and Adults with Incapacity Act.
This post is a full time post comprising 10 sessions, of which 9 are for clinical work and 1 for supporting professional activities. The appointee will join the on call rota for mental health services for North West Glasgow area of the health board. The on call frequency is 1:30, with a 3% availability supplement payable.
Applicants must have full GMC registration, a licence to practise and be eligible for inclusion in the GMC Specialist Register for Old Age Psychiatry. Applicants must be on the Specialist Register or be within 6 months of the anticipated award of a CCT or CESR(CP) at the time of interview for the post.

Job Description
1 Old Age Mental Health Service
Mental Health Services for Older People in Greater Glasgow and Clyde are currently delivered by multidisciplinary, consultant led teams, each serving an elderly population of approximately 10,000 (for a full time consultant). Assessment beds for both functional and organic mental illness are located on six hospital sites; Vale of Leven, Royal Alexandria, Inverclyde Royal, Stobhill, Gartnavel Royal and Leverndale. There are plans within NHS GGC 5 year plan, to reduce the number of acute admission sites.
There is a Clinical Director for Old Age Psychiatry (Dr Elizabeth Quinn) who takes a strategic overview of the whole service.
All community services within Greater Glasgow are now re-organised into 6 HSCPs; Glasgow City, West Dunbartonshire, East Dunbartonshire, Renfrewshire, East Renfrewshire and Inverclyde.
2 Old Age Clinical Services
Inpatient Services – Dumbarton, Alexandria, Helensburgh and the LochsideAcute inpatient services are provided at the Vale of Leven Hospital and there are 18 admission beds. These comprise 2 assessment wards; 1 for elderly people with functional illness and 1 for assessment of people with dementia.
Psychiatric Acute Liaison TeamThe area is served by the Vale of Leven Hospital and the Royal Alexandria Hospital (RAH) but the post holder will not be expected to provide any input to the RAH which has its own dedicated liaison team. Liaison assessments at the Vale are carried out by the specialty grade psychiatrist however the post holder will have time in his or her job plan for supervision and occasional assessments. Laboratory, X-ray and scanning facilities are provided by the acute hospitals.
Composition of the Community Mental Health TeamsThe successful candidate will work with members of 1 multidisciplinary Community Mental Health Team which covers Dumbarton, Alexandria, Helensburgh and the Lochside. The team comprises;1 band 7 nurse team leader (for Dumbarton and Clydebank) 2 Band 6 CPNs, 4 band 5 CPNs, 4 Band 3 Nursing assistants, 1 band 6 Occupational Therapist, 1 band 5 Occupational therapist, 1 Occupational Therapy Technician, 1 physiotherapist (shared with Clydebank), 1 CBT CPN (shared with Clydebank and PCMHT) and 1 Consultant clinical psychologist (shared with Clydebank) and 1 clinical psychologist (shared with Clydebank), There is a band 6 CPN for the care homes, with cross cover arrangements with the band 6 care homes CPN for Clydebank area.
Social Work Services are not part of the team, but work closely alongside the community staff.
Outpatient FacilitiesThe post holder will run outpatient clinics in Dumbarton Joint Hospital and Helensburgh Victoria Infirmary.
West Dunbartonshire HSCPThere are well established links with Primary Care, Geriatricians based at the Vale and Social Care Staff.
3 Supporting Staff
MedicalPsychiatric care in Greater Glasgow and Clyde is consultant led and supported by input from Psychiatry trainees, FY2 and GP trainees, and Specialty Doctors. The post has a full time and experienced specialty grade psychiatrist. The job plan for the Specialty Grade Psychiatrist include community and acute hospital liaison responsibilities however the post holder will be able to make changes to the job plan if required.
ClericalThe post will have secretarial and administration support. Clerical support is provided by a medical secretary and 2 team secretaries all based in Dumbarton Joint Hospital. The wards have a part time clerkess based on the ward. PsychologyWorking across the 2 West Dunbartonshire Older Adults services are a Full time consultant Clinical Psychologist, a Clinical Psychologist and a nurse trained in CBT, providing psychological based therapies and neuropsychological assessments.

5. Service Developments
Liaison services to the care homes have been developed over the past few years and the service is provided by one highly experienced Band 6 CPN with some support from a band 3 Health Care Assistant.
6. Duties of the Post
This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.
Suggested Job Plan for a Full Time Consultant
In terms of sessional commitment, this is envisaged to be roughly as follows:

The successful consultant will take part pro rata in the west on call rota 1 in 30 at present covering both the Vale of Leven and Gartnavel Royal sites.
7 Teaching
a) Undergraduate There is undergraduate teaching carried out during, at present, two terms of the academic year and students are attached from Glasgow University. The undergraduate course is currently being restructured and the post holder would hopefully have some involvement in the teaching of undergraduates.
b) PostgraduateThe post holder will participate in local Postgraduate Training Scheme in Psychiatry. These schemes are fully accredited by the Royal College of
Psychiatrists. She/he will participate in the educational programme of journal clubs and case conferences
c) Other DisciplinesContinued training of other staff would also constitute a part of the post holder’s job.
d) CarersThe post holder will be expected to participate in the education of carers on a formal and informal basis.
e) Continued Professional DevelopmentCPD has a high profile in Glasgow Psychiatry and a regular programme of activities has been organised. The post holder will be strongly encouraged to make use of these.
8 Research
Glasgow Mental Health Services for the Elderly have an excellent track record of clinical research. With 2 North West consultants having dedicated research sessions within ther job plans, the post holder will be ideally placed to advance his or her research interests.
9 Clinical Audit
The Mental Health Division is particularly keen on clinical audit that establishes effectiveness of interventions. The post holder will be expected to play a full part in this and develop ideas for audit projects on a multidisciplinary basis.
10 Management and Committee Work
The post holder will be members of North West Division of Psychiatry and the Glasgow Division of Psychiatry for older adults. Medical Management is through Sector Management structures.

Model Job Plan
Consultant Job Planning
1. A model job plan is attached, which has been agreed as the format used by employers in Scotland to record details of doctors’ or dentists’ working arrangements under the new contracts.
2. It should be used to confirm the outcome of both annual and interim Job Plan Reviews and the details contained in the Job Plan will form part of the contract between employer and employee. Doctors or dentists participating in job planning activities should be reminded of the important nature of this document.
3. The document must show the total number of Programmed Activities (PAs) (including those allocated for on-call work) and the number of Additional Programmed Activities (APAs) separately.
4. To ensure clarity and consistency, each item of work should be categorised as Direct Clinical Care (DC), Supporting Professional Activity (SPA), Additional NHS Responsibilities (AR) or External Duties (ED) and each should be listed separately as either a PA or an APA.
5. It should be noted that some blocks of time may include more than one category of work, but must not include both PAs and APAs. These must be noted separately to inform any APA contract that results from the job planning process.
6. It is recommended that a diary should be completed as an evidence gathering tool to inform the process. To ensure that a representative period is used, it is recommended that diary evidence should be submitted for a minimum period of six consecutive weeks or, where practicable, the period of one cycle of on-call.
7. The reference period used for the diary should be agreed in advance between the doctor or dentist and their clinical manager.
