1. Introduction Women’s Health and Perinatology Research Group has been working actively in research and education since
its establishment in 2009 with the aim of enhancing health and wellbeing of women and babies. We focus on
clinical, basic and translational researches as well as epidemiological studies related to obstetrics, maternal-
fetal medicine, perinatal medicine, female incontinence and reproductive medicine.

Ongoing project description
Ganesh Acharya
His research activity mainly focuses on the investigation of fetal and maternal cardiovascular changes in
physiological and pathological pregnancies. The focus is on the translational aspect utilizing experimental
animal models as well as clinical material and epidemiological data in our research. Some of the ongoing
projects are outlined below.
Experimental studies on maternal and fetal cardio-vascular function using animal models
This is an ongoing collaboration with Prof Juha Räsänen (University of Helsinki, Finland), Oregon Health
Sciences University, Portland, USA and University of Oulu, Finland for more than 16 years. Currently we
are conducting experiments on fetal sheep to study fetal cardiovascular function and myocardial
remodelling under different loading conditions using conventional echocardiography, myocardial tissue
Doppler imaging, ultrasonic flow probes and pressure volume conductance catheters in vivo. We are also
investigating the feasibility of applying new echocardiographic techniques such as colour tissue velocity
imaging in diagnosing fetal hypoxemia/acidemia. We are testing the possibility of using certain
pharmaceutical preparations to increase fetal left ventricular preload by improving fetal pulmonary blood
flow, which may be useful in preventing/ameliorating cerebral hypoxemia in intrauterine growth
restricted fetuses.
Genetic and molecular markers of feto-maternal diseases
We are attempting to identify, characterize and clinically validate novel markers of preeclampsia and
placental and decudial dysfunctions.
Noninvasive assessment of cardiac function and systemic blood flow in human fetuses: An observational
study
This project is conducted in collaboration with Karolinska Institutet, Karolinska University Hospital and
Royal Technical Institute, Stockholm. In this project we plan to establish longitudinal reference ranges for
the parameters of heart function measured using tissue velocity imaging (this part of the study is
performed in Tromsø), apply this technique to identify fetuses at risk of complication due to hypoxemia
(IUGR, postdate pregnancy) and volume-overload (anemia due to red cell alloimmunization, twin-twin
transfusion syndrome), and develop an automated and easy way of measuring relevant parameters of
cardia function.
Maternal-fetal hemodynamics, endothelial function and placental blood flow
This is an ongoing large study evaluating maternal hemodynamics and endothelial function together with
several biomarkers to identify women at risk of pregnancy complications. Placental and fetal blood flow is
also simultaneously investigated. We are also studing sex diffrences in fetal and placental blood flow using
data collected in this study and previous studies. We have a biobank of blood samples and placental
samples collected from approximately 700 participants.
Long-term health and wellbeing of women with a previous history of pre-eclampsia
In this project we utilize the data from a large longitudinal population based-survey of Tromsø municipality
which has been ongoing for >40 years. Our main focus has been cardiovascular health of women with

previous history of preeclampsia. The project is conducted in collaboration between our research group
and the Institute of Community Medicine, University of Tromsø.
Longitudinal changes in oral microbial milieu in normal pregnancy and pregnancy outcomes
Oral health in pregnancy is a neglected area of research. In this study in collaboration with the Institute of
Odontology in Tromsø we are investigating the longitudinal changes in oral microbial milljo during
pregnancy and their relation with pregnancy outcomes.
New nano-drug delivery systems for the treatment of vaginal infections
In collaboration with the “Drug-Delivery” Research group at the Department of Pharmacy, we are
developing new liposome-based nanodrug formulations for local delivery of intimicrobials and
antiinfammatory agents to the genital tract. One PhD thesis has been completed recently and one
postdoctorial project has been just started. The postdoctorial candidate will spend one year at Karolinska
Institutet.
Impact of free preconception care in rural China
In collaboration with Fudan University, Shangahi, China, we are utilizing a very large national dataset
collected during the implimentaion of National Free Preconception Health Care Project in China in selected
rural counties and some municipalities covering all 31 provinces in the country to study preconception risk
factors, vaccination status prevalence of TORCH infections and impact of free preconception care on
pregnancy outcomes. One PhD student financed by Fudan University is registered at UiT.

Maternal death in Norway
This is a national project on maternal death from the last 20 years and up-dated every year, also in
collaboration with other Scandinavian countries. Project leader Siri Vangen, The National Hospital,

Labor progression study (LaPS study)
This is a cluster-randomized study in 14 Norwegian hospitals testing out two different partograms. In
Norway and most other countries the Friedman’s partogram for labor progression has been used.
Contemporary research by Zhang shows that the dilatation of the cervix can be substantially different,
especially in the early stage of labor and a new partigram is developed. The type of partogram used is
important for the diagnosis of labor dystocia.

In this study more than 70000 women are included and the inclusion process stopped 31 January 2017.
The primary outcome is cesarean section. Secondary outcomes are indications for cesarean section, the
rate of oxytocin augmentation, total duration of augmentation, duration of labor (first and second stage),
rate of dystocia, operative vaginal deliveries, bleeding > 500ml, Apgar score ≤7 at 5 minutes, metabolic
acidosis, transfer to NICU and women’s experience with birth care. Data are now being analyzed. Project

Semen quality and oxidative stress
Semen quality has been found to be deteriorating trend at large. One of the causes has been pointed to
the oxidative stress. A PhD project is developed to study semen quality among the healthy population and
men needed infertlty treatment in the Northern Norway and their root cause of deterioration together
with oxidative stress. Project leader Purusotam Basnet is working with collaborators, Ganesh Acharya and
Natasa Skalko Basnet.
Spermatozoa motility and viability
Different supplements in the media to increase in vitro motility and viability of sperm cells have been
screened. Project leader Purusotam Basnet is working with collaborator Natasa Skalko Basnet.
Oxidative stress in human embryo preimplantation stage
A preliminary method for the measurement of oxidative stress by using the tools and technology of
Nanoscopy in the pre-implanted human embryo is under development.
Subcellular anatomy of preimplanted human embryo using nanoscopy
This is a part of the project funded by UiT Strategic Grant for the establishment of Centre for Advanced
Nanoscopy to Decode Sub-cellular Biological Systems. This work package is approved by UiT for the PhD
research with Principal supervisor Purusotam Basnet and co-supervisors Ganesh Acharya, and Balpreet
Ahluwalia.
Norway-India Project
This is an international project between Norwegian partners (Department of Physics University of Tromsø,
University of Tromsø, University Hospital of North Norway, Tromsø) together with Indian partners (Indian

Institute of Technology, New Dehli, India and other institutions). The grant is supported by Norwegian
Research Council. Main objectives of the study are to develop nonlabeling methods to study subcellular
structures using nanoscopy technique.

Lactation, Menopause and Bone Health
The research on Lactation, Menopause and Bone Fragility based on data collected during her post-doctoral
in Melbourne, Australia is ongoing, with focus on 3D architecture of bone - to increase our understanding
of the pathophysiology behind bone fragility.
Pathophysiology and Diagnosis – Bone structure study
An extention of the Melbourne work after returning to Tromsø is a unique opportunity to test hypothesis
on diagnostic tools for assessment of fracture risk by using a relatively new software developed in
Melbourne for analysis of CT images, based on fracture cases and controls in the population-based Tromsø
study. This research focuses on pathophysiology of bone fragility, and diagnostic tool to better identify
those who are in need of treatment.
Secondary prevention of Fracture
NoFRACT – Norwegian Capture the Fracture Initiative is a huge national study involving secondary fracture
prevention. All women and men with fractures at seven Norwegian hospitals are identified, offered
examination and treatment for osteoporosis. The effect of introducing this intervention on re-fractures
rates and mortatlity will be measured using national registers during 2008-2020.

A quality assurance study include women with and without diabetes during gestation in Northern Norway,
for assessment of complications of diabetes, and factors associated with complications.

Prenatal diagnosis of abnormally invasive placenta (AIP) by ultrasound and MRI.
I am one of the lead investigators of a multicenter prospective study involving 20 centers in Italy aiming
at ascertaining the degree of association of different ultrasound and magnetic resonance imaging signs in
detecting the presence of AIP from the first trimester onwards. The data collection started last September.
Estimated completion date has been set in December 2017
Placental insufficiency and intra-uterine growth restriction.
I am part of the Truffle-2 Study, a multicenter prospective study involving several centers in Europe aiming
at ascertaining the role of fetal Doppler in identifying fetuses affected by late onset intra-uterine growth
restriction at higher risk of perinatal compromise.

Multiple pregnancies
I am the lead investigator of the STORK group, a research collaborative including 9 hospitals in the South-
west region of Thames. Several studies have already been published in the last two years as part of my
PhD thesis. The data analysis is continuing; the main topic of the future publications will be perinatal
outcome of twin pregnancies after one twin death, perinatal morbidity in twin pregnancies affected by
birthweight discordance and perinatal outcome in high order multiple gestations.
Pre- and post-natal fetal Doppler
I am the lead investigator of a large prospective study carried out in Italy on the correlation between pre
and post-natal Doppler in uncomplicated preganncies and in those affected by intra-uterine growth
restriction.

4. Education
Teaching in obstetrics and gynecology (women’s health) is demanding especially since bed-side teaching must
be given in a one-to-one basis (in the obstetric out-patient clinic, the gynecological out-patient clinic, the labor
ward, gynecological and gyn-oncology units, and in the operation theater) rather than to groups of students.
Teaching for 4th year medical students in obstetrics and gynecology has been offered as the international
semester with 30-40 internatioanal students in English medium. There were 36 students participating this year
including for lectures, clinical training and practice.
Medical students (MED-3501: 5th year) have clinical practice at Department of Obstetrics and Gynecology,
UNN. Every student was given clinical practices and training in obstetrics and gynecology for 2 weeks.
Since autumn 2017, we increased the number of Seminars for the 6th year medical students. We introduced
“Test-Exams” for each of 6th year students in a one to one patient setting, followed by exam questions in
groups. This module has been very popular among the students.
