    @mp 3on3Voluntary Waiver and Release AgreementThis Voluntary Waiver and Release Agreement (Agreement) is hereby executed by the undersigned, or if theundersigned is under the age of 18 years, then the undersigneds parent or legal guardian (together with any heir,successor, representative or assign, collectively as Participant) in favor and for the sole and exclusive benefit ofthe Organizers (as defined herein). In connection with Participants involvement in a contest, promotional activityor in any other event which may take place at Celebration of Praise Church of God or Amplify Youth Ministries or

    Amp3on3 or which involves the use of Celebration of Praise Church of God or any of the facilities at the

    Celebration of Praise Church of God campus by Participant including, without limitation, a 3on3 basketballtournament and 3-point contest, Participant hereby certifies, warrants, represents, agrees and covenants toCelebration of Praise Church of God and Amplify Youth Ministries and any of the respective Event sponsors orpromoters, including, without limitation, all charities benefiting from the Event (individually and collectively,together with each of their respective affiliates, officers, employees, partners, members, board members,

    sponsors, volunteers, tenants, contractors, agents, successors and assigns, the Organizers), that he/she is:

    (1) FREE OF ANY MENTAL OR PHYSICAL CONDITION, AILMENT OR INJURY (MEDICAL OR OTHERWISE) WHICHWOULD, IN AND OF ITSELF OR IN CONJUNCTION WITH ANY OTHER CIRCUMSTANCE, INCLUDING BUT NOT

    LIMITED TO THOSE ACTIVITIES ASSOCIATED WITH THE EVENT, (i) IMPAIR, PREVENT OR PROHIBITPARTICIPANT FROM ENGAGING IN SUCH EVENT ACTIVITIES OR (ii) BE AFFECTED,

    AGGRAVATED OR WORSEN IN ANY WAY AS A RESULT, DIRECTLY OR INDIRECTLY, OF PARTICIPANTSINVOLVLEMENT IN SUCH EVENT;

    (2) OF SOUND MIND AND BODY AND NOT UNDER THE INFLUENCE OF ALCOHOL OR ANY ILLICIT OR

    PRESCRIPTION DRUG OR MEDICATION WHICH MAY IN ANY WAY IMPAIR PARTICIPANTS ABILITY TO ENTERINTO THIS AGREEMENT, FULLY UNDERSTAND THE RESPECTIVE INTENT AND MEANING OF ALL OF THE TERMSAND PROVISIONS HEREOF AND TO PARTICIPATE IN THE EVENT;

    (3) ENTERING INTO THIS WAIVER AND RELEASE AGREEMENT VOLUNTARILY, BY PARTICIPANTS OWN FREEWILL, ACT AND DEED, WITHOUT ANY UNDUE INFLUENCE FROM THE ORGANIZERS OR ANY OTHER THIRDPARTY.PARTICIPANT AGREES THAT HE/SHE DERIVES A MATERIAL BENEFIT FROM THE EVENT AND/OR

    PARTICIPANTS INVOLVEMENT THEREIN. BY EXECUTION OF THIS AGREEMENT, PARTICIPANT AGREES ANDACKNOWLEDGES THAT A STRENUOUS ACIVITY, SUCH AS BASKETBALL, IS POTENTIALLY HAZARDOUS, AND

    PARTICIPANT HEREBY ASSUMES ALL RISKS ASSOCIATED WITH PLAYING IN THE EVENT, INCLUDING, WITHOUTLIMITATION, FALLS, CONTACT WITH OTHER PARTICIPANTS, THE EFFECTS OF WEATHER AND THE CONDITION

    OF PLAYING SURFACES. FURTHERMORE, PARTICIPANT EXPRESSLY AND UNCONDITIONALLY ASSUMES ALLRISKS AND DANGERS KNOWN OR UNKNOWN, FORESEEN OR UNFORESEEN, AND RELATING OR INCIDENTAL TO

    PARTICIPANTS INVOLVEMENT IN THE EVENT AND ANY ACTIVITY ASSOCIATED THEREWITH. PARTICIPANTHEREBY RELEASES, FOREVER DISCHARGES AND HOLDS HARMLESS THE ORGANIZERS FROM AND AGAINST ANY

    AND ALL CLAIMS, DAMAGES, LIABILITIES, COSTS AND EXPENSES, INCLUDING, BUT NOT LIMITED TO, DEATH,PERSONAL INJURY OR PROPERTY DAMAGE OF ANY KIND OR NATURE, ARISING OUT OF OR RELATING TO

    PARTICIPANTS INVOLVEMENT IN THE EVENT AND ALL ACTIVITIES ASSOCIATED THEREWITH.Each Participant understands that by signing this Agreement, each Participant covenants and agrees that suchParticipant, as well as such Participant's heirs, executors, administrators, successors and assigns, will neverinstitute any suit or action at law, or otherwise, against any of the Organizers, or in any way aid in the institutionor prosecution of any claim, demand, action or cause of action for damages, costs, loss of services, expenses orcompensation for or on account of any damage, loss or injury either to Participant's person or property, or both,which may result from the Participant's participation in the Event or any other Event associated therewith. EachParticipant agrees to defend and hold the Organizers harmless from any loss or damages, including attorneys' feesand costs, sustained by Organizers, or any one or more of them, as a result of any Participant's recession of this

    Agreement or breach of its covenants or agreements contained in this paragraph.Participant further agrees that the Organizers shall have the right to record, broadcast and otherwise exploit in any

    and all media throughout the world Participants activity/performance in the Event and to use Participants name,likeness, voice and biographical information in connection therewith. If any provision of this Agreement shall forany reason be held invalid or unenforceable, such invalidity or unenforceability shall not affect any other provisionhereof and this Agreement shall be construed as if such invalid or unenforceable provision were omitted.THE WAIVER AND RELEASE GRANTED BY PARTICIPANT HEREUNDER IS LEGALLY BINDING AND SHALL BECONSIDERED IRREVOCABLE. PARTICIPANT AGREES THAT THE ORGANIZERS MAY RELY UPON THIS AGREEMENTTO THE FULLEST EXTENT PERMISSIBLE AT LAW OR IN EQUITY.

    Amp 3on3 Rules & RegulationsSPORTSMANSHIP POLICY

    Good sportsmanship and cooperation is both anticipated and expected. Theteam captain is expected to aid in controlling teammates and team followersconduct and to represent his/her team as spokesperson in case of appeals to the

    court official. Poor sportsmanship could result in penalties against the team ineither the Technical, Intentional, or Flagrant foul categories (see rule 9). Failureto comply with the Spirit of the Amp 3on3 Basketball Tournament, with acts suchas "foul language including but not limited to cursing, fighting, taunting,intimidating or verbally attacking a tournament official, player or spectator maylead to removal of that player, team and/or spectator from the tournament. Anyejection from the entire tournament will result in a suspension from the nextyears Amp 3on3 Basketball Tournament, at a minimum. Decisions made bytournament officials are final, and are not subject to review by video or otherrecordings, or other sources. If you are ejected from the tournament, there is norefund of registration.

    1 Who Can PlayThe tournament is open to players in 3 categories: Middle School, HighSchool, as well as adults.

    2 Number of Players for 3on3

    Each team may consist of a minimum of 3 players (plus 1 alternate in the caseof injury or scheduling conflict) *Note: the team will only win rewards for 3players. A team may have only 3 players on the court at any time. All gamesmust start with at least 3 players on each team.

    3 Authorized Equipment & Apparel

    No player shall be allowed to wear a guard, cast, hard brace or otherpotentially dangerous equipment on his or her elbow, hand, wrist, or forearm,including equipment made of hard leather, plastic, plaster or metal, even if theequipment is covered with soft padding. Soft braces, sleeves and wraps will beallowed unless they pose a danger to other players. The Amp 3on3 BasketballTournament retains the right to disallow any equipment or apparel that it

    judges to be dangerous or inappropriate.

    4 Eligibility Issues

    All high school and college players are encouraged to check with theircoaches and/or athletic directors about participating in the Amp 3on3Basketball Tournament and 3-pont contest, and the potential effects on their

    eligibility. The Amp 3on3 Basketball Tournament is not responsible fordetermining a particular athletes eligibility under National Collegiate AthleticAssociation or applicable high school association rules.

    5 Category Types

    The Amp 3on3 will have a 3on3 and 3point competition for Middle school, HighSchool and Adults to participate in. False information will be grounds fordismissal from the tournament. Each player in all brackets must provide copiesof photo identification with registration. Failure to do so may result indisqualification from the tournament.

    6 Basket Height

    Basket height will be 10 feet high for all brackets.

    7 Basketball SizeThe full size ball will be used for all brackets of competition.

    8 Stealing the BallPlayers in all age categories may steal the ball when it is being passed.

    9 Fouls

    Basketball officials will referee all games. All called fouls, whether shooting ornon-shooting, shall result in one free throw shot, except on a successful fieldgoal, in which case the basket counts and no free throw shot is awarded.However, if a player is beyond the two-point arc and is in the act of shootingwhen fouled, two free throw shots will be awarded, except on a successfulfield goal, in which case the basket counts and no free throws are awarded. Achange of possession will result regardless of whether the free throw shot(s)is (are) made or missed. During a free throw, opposing team players may notintentionally disrupt the shooters unhindered throw. After a foul shot, the ballwill be placed into play from the backcourt line. Incidental contact betweenopponents shall not result in a called foul unless such contact results in ameaningful disadvantage. The following fouls will result in additionalsanctions:

    A Technical FoulsA technical foul will be called for unsportsmanlike acts such as taunting,baiting, or trash talk. Taunting and baiting can involve derogatory remarks orgestures that incite or insult a player. Trash talk involves a deeply personal,verbal attack directed toward any person involved in the event. In extremecases, the player may also be suspended from play and a coach or fanremoved from the court for the remainder of that game or for the rest of thetournament. A player who aggressively comes into contact with or assaults acourt official, court marshal, or other tournament official shall be automaticallyejected from the game and for the remainder of the current tournament.

    Additional suspension for such acts will be determined by the Amp 3on3Tournament on a case-by-case basis. The court official may also assess atechnical foul if the official determines that the team is stalling in the interest ofpreserving a winning margin. A technical foul results in one point for theoffended team and possession of the ball.

    B Intentional FoulsAn intentional foul is a foul designed to neutralize an opponents obviousadvantageous position. It is a foul which, based on the officials observation ofthe act, is not a legitimate attempt to directly play the ball. A foul shall also beruled intentional, based on the officials observation of the act, if whileattempting to play the ball, a player causes excessive contact. An intentional

    foul results in one point for the offended team and possession of the ball.C Flagrant Fouls

    A flagrant foul may be of a violent or aggressive nature, or an act, whichdisplays unacceptable conduct. It may or may not be intentional. It mayinvolve violent or aggressive contact such as striking, kicking, kneeing,moving under an opponent who is in the air, and crouching or hipping in amanner, which could cause severe injury to the opponent. It may also involvedead ball contact or dialogue which is extreme or persistent, aggressive, orabusive. A flagrant foul results in one point for the offended team and

    possession of the ball. The player committing the foul could cause the team tohave to forfeit the game if and the rest of the tournament if the situation getsout of hand and peoples safety is put at risk.

    10 BoundariesThe basket structure, padding, and structural supports will be played as out-of-bounds. The actual backboard, including its face, top, bottom and sides,shall be considered in-bounds.

    11 StallingStalling is prohibited. Stalling is a style of play in which a team does notactively attempt to advance the basketball toward the basket and shoot theball at the basket. It is a method used in an attempt to run out the game clockto preserve a win. Stalling is considered an unsportsmanlike act and willresult in a technical foul against the offending team (see rule 9a).

    12 Which Team Receives the Ball First?

    A coin toss prior to each game will determine which team gets the ball out-of-bounds first.

    13 Keeping Score

    All made baskets from inside the two-point arc count for one point and made

    baskets from outside the two-point arc count for two points. The first team to

    21 points or whoever leads at the end of the game is the winner. (See rule 14

    for exceptions). (Except for championship in each category)

    14 Length of Game All DivisionsIf a score of 21 is achieved before regular game time expires than the gameis over. The target score (except for the championship game) for all games is21 points, meaning the first team to reach 21 points within 16 minutes of playis declared the winner. The team that reaches 21 points does not need tohave a winning margin of 2 points or greater. The 16-minute clock is stoppedif the court official stops play for a player injury (see rule 22), if a team usestheir 1-minute time-out or other unusual circumstances. If neither team has

    reached a score of 21 points, the court official shall stop the game after 16minutes of play.

    In all situations, the court official shall declare a technical foul (see rules 9a &10) if the official determines that a team is intentionally stalling to run out theclock.If a score of 21 is not achieved within 16 minutes:If at this point of interruption a team has a lead of any # of points, that team isdeclared the winner. If neither team has a lead, the overtime rule will beactivated. In overtime, the first team to score during the overtime session willbe declared the winner. A coin toss will determine who gets the ball out ofbounds first in overtime.(All is the same for the championship round except the game time will include

    2 - 15 minutes halves with a 5-minute half-time break.)

    15 Checked Ball

    The ball must be checked behind the 2 point arc at the top of the key by anopposing player before it is put into play if possession changes.

    16 Change of Possession

    The ball will change possession after scored baskets and all free-throw

    attempts with the exception of Technical, Intentional or Flagrant fouls (see

    rule 9 a, b, c). There will be no make it, take it rule.

    17 Taking It Back

    The ball will be taken back on each change of possession, regardless ofwhether or not a shot was attempted. Failure to take it back results in loss ofpossession and any points just scored. Taking it back means bringing yourwhole body and the ball behind the 2 point arc at the top of the key.

    18 Ball Out-of-Bounds

    A ball out-of-bounds will be taken out from the court line where it went out.

    19 Jump BallIn a jump ball situation, the ball will first go to the team which gainspossessions thereafter.

    20 DunkingDunking is allowed however, if a player hangs on the rim in any way it willresult in a technical foul. If a player hangs on the rim again their team willforfeit that game. (For the safety of players and protection of churchequipment this rule will be strongly enforced)

    21 Substitutions

    Substitutions may only be made if 1 of the 1st 3 players registered can notplay due to injury or scheduling conflict and must be approved by Amp 3on3officials.

    22 Time-Out

    Each team is allowed a single one minute time-out per game. The clock willstop running during a time-out.

    23 Player InjuryA court official has the discretion to suspend play for the protection of aninjured player. If a player is bleeding or has an open wound, that player willbe directed to leave the game and properly bandage the wound. A playerwith any bloodstained clothing or bandage must remove the stained orsaturated material prior to re-entering the game. If it is believed that a player

    has lost consciousness during a game, or is severely injured, the team willhave to forfeit their game unless a 4 th alternate player has been registeredand is available to continue as the 3rd team-mate.

    24 Game TimesFirst teams to play will be drawn randomly. Each team is required to checkthe Master Scoreboard for all official schedules, times, courts, revisions, andgeneral game information. Teams must be ready to begin play at theirscheduled start time. Teams not at their court for their scheduled game aregiven a 1 minute grace period before a forfeit is enforced. Teams areencouraged to remain in contact with their court to observe the effects ofscheduling changes that could occur. Non-playable situations may result in

    the modification or cancellation of the event. Under these unlikelycircumstances team entry fees will not be refunded but a rescheduled datewill be made available for the event.

    25 The Fine PrintDesignated tournament officials shall have the power to make decisions onany points not specifically covered in the Rules and Regulations and shallalso have the complete authorization to interpret the intent and purpose ofthese Rules and Regulations. The Amp 3on3 Basketball Tournament officialsalso reserve the right to disqualify any player and/or team for infractions oftournament policies, including the following:

    Use of Illegal Players:The players listed on the team entry form as accepted by the Amp 3on3basketball Tournament are the only ones eligible for play on that team.Player changes must be registered accepted by tournament officials andapproved by 8:00 a.m. on Wednesday, October 12, 2011. Under nocircumstances will roster substitutions be allowed after the tournamentbegins. Any team using a player not properly registered will be disqualifiedfrom the tournament.False Information:

    Information provided on a team registration is the basis for bracketscheduling and is expected to be both accurate and complete. Players listing

    inaccurate information on these forms may be disqualified from the

    tournament with no refund.
