CHAPTER 1 INTRODUCTION

1.1 Introduction:

Medical department of Jaipur Division is headed by Chief Medical

Superintendent. Divisional Hospital at Jaipur has been upgraded to Central Hospital,

and is headed by Medical Director. There are two Sub Divisional Hospitals namely

Bandikui & Rewari and six Health units namely, Alwar, Sikar, Ringus, Phulera, Gator

Jagatpura and NWR HQ-JP. Further there are three Scale-I ARMEs available over

the Division ie. SPARMV at Jaipur, ARME Scale-1 (BG) at Reweri, and ARME Scale-1

(MG) at Sikar.

Divisional Hospital, Jaipur, has been upgraded to Central Hospital, NWR. There are

2 Sub-Divisional Hospitals situated at Rewari, and Bandikui. We provide medical

care to about 52,350 Railway population spread over a vast area. There are 11108

employees and 3959 Retired Senior Citizen in this Division.

1.2 Organization & Facilities:

Medical Services of Jaipur Division consist of Central Hospital at Jaipur City

assisted by nine Health Units, of which three are situated in Jaipur City itself.

CHAPTER 2 ACKNOWLEDGEMENT

2.1 Coordinating Officials of the department:

2.2 Terms of Reference:

This study has been conducted under the following terms of reference:

i) Review of staff vis-a-vis workload on existing working conditions.

ii) Suggesting the ways and means for improving the system economically and

efficiently.

2.3 Methodology Adopted:

The following techniques of method study as well as work measurement have

been applied to conduct the study:-

i) Data collection and its critical analysis to arrive on factual status of present

working.

ii) Over view of work outsourced against the existing man power.

iii) Holding discussions at various levels with a view to produce fruitful results.

iv) Sample checks and spot observations.

2.4 Field units visited:

Work study team has visited the Central Hospital, Jaipur for receiving relevant

circular, policy, yardstick/benchmarking and data. Health units and colonies at

stations of JP, BKI, AWR, RE, FL, KSG, RGS & Sikar were also visited by the

team to observe the working conditions and procedures being adopted by the

staff to perform the work.

CHAPTER 3 SYNOPSIS

At present Safaiwala/Safaiwali staff of Medical department are deployed over 15

units of Jaipur division for sanitation work of Railway colonies, hospitals and health

units.

Outsourcing for sanitation work is already initiated at Jaipur against the vacancies.

These contracts are more cost effective and running successfully.

Therefore, Medical department is planning to outsource more work against the

existing vacancies of the man power.

As such the sanitation work has been outsourced successfully against the vacant

posts in the cadre. Keeping all the above mentioned factors in view, a review on

the present requirement of staff has become essential.

To achieve the goal, a work study was conducted by the work study team in close

coordination with nominated coordinator and other staff of the department which is

discussed in the subsequent chapters.

44.3 Duties of Safaiwala/Safaiwali: Duties of Hospital/Health Safaiwala/Safaiwali

and Sanitation Safaiwala/Safaiwali are defined separately in medical manual.

Descriptions of their duties are as under:

4.3.1 Duties of Hospital/Health Unit Cleaner (Safaiwala/Safaiwali) are as under:

i) Sweep the road, ground, floor etc. of the hospital/health unit.

ii) Frequently clean the floor of the wards with wet cloth and clean the hospital

doors, windows, window panels etc.

iii) Clean the bed pans, sputum cups, urinals commodes and latrines attached to

hospital/health unit.

iv) Supply bed pans and urine bottles to the patients and clean them after use.

v) Carry the patients on stretchers in the hospital and from station to hospital

and vice versa.

vi) Carry the hospital stores.

vii) Wash the soiled clothes.

viii) Carry out any other order given by his/her superiors

4.3.2 Duties of Sanitary/Conservancy Sweeper/Cleaner/Safaiwala/Safaiwali are as

under:

i) Collect, remove and dispose of rubbish from Railway premises and colonies

etc.

ii) Sweep the road and other surface area of the Railway premises and colonies

and collect the sweeping and put these in the dustbins and also dist and

remove cobwebs etc.

iii) Clean the drains and carry out periodic de-silting.

iv) Undertake all other activities related to cleaning of colonies/Stations.

v) Carry sanitary stores.

vi) Carry out any other duty as ordered by the Health Inspector and other

Medical officers.

8.1.6 Suggestion if any:

Process for comprehensive contract of general

cleaning, civil engineering works & lawn and

maintenance of electric equipments of Central Hospital

has been initiated. Presently 17 Safaiwala staff are

working in the Central Hospital against the sanctioned

strength of 22 posts. Further 02 Safaiwala are going to

retire in 2019 and 03 Safaiwala staff are habitual

absentees. Thus, effective working man powers will

remain 12 only. Coordinator Shri Deepak Narolia,

CHI/Central hospital, JP has informed that after

outsourcing of cleaning work surplus staff will be used

for cleaning of circulating area of Central Hospital.

8.1.7 Observation:

Work study team with a view that when complete cleaning work of Central Hospital

is being outsourced, then there is no need to hold departmental Safaiwalas for

cleaning of circulating area. This kind of geographical segregation between two

agencies for same work causes dispute. Therefore, it is suggested that circulating

area of Central Hospital should also be incorporated in outsourcing. Hence, all the

22 posts of Safailwala have been considered surplus. It is being advised that 05

vacant posts may be surrendered forthwith and balance 17 posts to be surrendered

after finalization of the cleaning contract.

1254 employees and 1816 retired employees are registered with Sub-divisional

Hospital, Rewari. The present average OPD is 140 and wards with 20 beds are

being operated with average 80% occupancy. There is separate building in the

premises for OPD patients. Out of sanctioned strength of 04 Safaiwala staff 03 staff

are working and among these 01 staff is going to retire on 31.12.2017. Thus,

effective man-power will remain only 02 through which cleanliness in the Sub-

Divisional Hospital, Rewari cannot be maintained round the clock. Hence, to ensure

good hygiene in the hospital the cleaning work must be outsourced.

9.1.0 Sanitation Safaiwala Staff under CHI/JP:

9.1.1 Brief description of Railway residential colonies in Jaipur city:

09 Railway residential colonies, RPF Barrack, GM & DRM’s Bungalows are situated

in deferent locations in Jaipur city. Out of these only 03 colonies are maintained by

departmental safaiwala staff.

9.1.5 Description of Contract work:

Period: Two years) This includes cleaning of Railway colonies –Panipatch,

New Loco & Microwave. Removal and disposal of garbage, kitchen waste,

house hold waste, all type of wastage (rubbish, debris, sludge, excavated

debris, mud-muck, stable litters) and refuse generated from these colonies,

hedges, garden refuse, small branches of trees, dry leaves, mowed lawn

grass, dead animals etc. from Railway colonies and dumping outside the limit

of Municipality at decided appropriate place.

(B) (Work order no. MD/120/15/Sanitation/PHOD/GTJT & DPA/2017-18; dated:

24.04.2017 for Period: Two years). This includes cleaning of Railway colonies

–Durgapura, Jagatpura & PHODs. Removal and disposal of garbage, kitchen

waste, house hold waste, all type of wastage (rubbish, debris, sludge,

excavated debris, mud-muck, stable litters) and refuse generated from these

colonies, hedges, garden refuse, small branches of trees, dry leaves, mowed

lawn grass, dead animals etc. from Railway colonies and dumping outside the

limit of Municipality at decided appropriate place.

9.1.6 Observation:

Sweeping and cleaning work in the Railway Colonies –Loco & Ganpati Nagar is

maintained by 29 staff out of which 13 Safaiwala staff are working at Loco Colony

and 12 staff are working at Ganpati Nagar Colony and 04 staff are used as rest

giver and leave reserve.

Against the sanctioned strength of 34 Safaiwala staff, 29 Safaiwala staff are

performing work and 05 posts are lying vacant. 02 Safaiwala are going to retire in

coming two years. Thus, effective vacancies will be 07.

It has already been tested in Ajmer division that work through outsourcing is much more

economical and effective as compare to work performed by the departmental staff.

Therefore, it is recommended that the cleaning work of Railway Colonies at Jaipur against

workload of 07 manpower should be outsourced. Hence, 07 staff of Sanitary

Safaiwala/Safaiwali have been considered surplus and advised to surrender forthwith.

Two years) Removal and disposal of garbage, kitchen waste, house hold waste, all

type of wastage (rubbish, debris, sludge, excavated debris, mud-muck, stable

litters) and refuse generated from the colonies, hedges, garden refuse, small

branches of trees, dry leaves, mowed lawn grass, dead animals etc. from Railway

colonies of Bandikui station and dumping outside the limit of Nagar Palika at

decided appropriate place.

9.2.6 Observation:

Sweeping and cleaning work in the Railway Colony of Bandikui is maintained by 18

staff out of which 11 Safaiwala staff are working for sweeping and cleaning, 03

Safaiwala staff are working for Drainage sewage work, 02 Safaiwala are deputed to

clean grass and veggies and 01 staff is being utilized as Khallasi in the office. This

work is being distributed and supervise by the Jamadar.

Removal and disposal work of the garbage is already outsourced vide work order

mentioned above in Para 9.2.5.

Against sanctioned cadre of 28 posts of Safaiwala/Safaiwali, 10 posts are lying

vacant and 04 staff are going to retire in coming 02 years (till June’2019).

It has already been tested in Ajmer division that work through outsourcing is much

more economical and effective as compare to work performed by the departmental

staff. Therefore, it is recommended that the cleaning work of Railway Colonies at

Bandikui against workload of 14 manpower (10+04) should be outsourced. So that,

14 staff of Sanitary Safaiwala/Safaiwali have been considered surplus and advised to

surrender forthwith.

9.4.6 Observation:

Sweeping and cleaning work in the Railway Colony of Rewari is maintained by 23

staff. Removal and disposal work of the garbage is already outsourced vide work

order mentioned above in Para 9.4.5.

Against sanctioned cadre of 33 posts of Safaiwala/Safaiwali, 10 posts are lying

vacant and 08 staff are going to retire in coming 02 years (till Oct ’2019).

If the cleaning work will be done through outsourcing, is much more economical and

effective as compare to work performed by the departmental staff. Therefore, it is

recommended that the cleaning work of Railway Colonies at Rewari should be

partially outsourced. Hence 18 (10+8) staff of Sanitary Safaiwala/Safaiwali have

been considered surplus.

It is recommended that 10 vacant posts may be surrendered immediately and 08

posts of on roll staff to be surrendered after finalized the contract and staff may be

redeployed at any required locations.

9.5.5 Observation:

Sweeping and cleaning work in the Railway Colony of Rewari is maintained by 16

staff. Removal and disposal work of the garbage is already outsourced vide work

order mentioned above in Para 9.5.4.

Against sanctioned cadre of 28 posts of Safaiwala/Safaiwali, 12 posts are lying

vacant and 04 staff are going to retire in coming 02 years.

If the cleaning work will be done through outsourcing, is much more economical and

effective as compare to work performed by the departmental staff. Therefore, it is

recommended that the cleaning work of Railway Colonies at Phulera should be

partially outsourced. Hence 16 (12+4) staff of Sanitary Safaiwala/Safaiwali have

9.8.6 Observation:

There is sanctioned strength of 05 Safaiwala against which 05 Safaiwala are performing work and

there is no vacancy in the cadre. One staff is going to retire in coming two years. Outsourcing in

small quantity is not successful and it becomes tough to monitor. Therefore all the 05 posts of

Sanitary Safaiwala at Sikar have been considered justified.

CHAPTER 10 CRITICAL ANALYSIS

10.0 Critical Analysis of Work load of Medical Safaiwala staff:

10.1 Hospital Safaiwala: In the sanctioned strength of 22 Hospital Safaiwala at

Central Hospital, Jaipur; 17 are on roll, 05 posts of Safaiwala are lying

vacant, 02 Safaiwala are going to retire in coming two years and 03 staff

are on long absent at Central Hospital, Jaipur. For newly constructed

building of Central Hospital a comprehensive contract for cleaning work,

civil work and electric work has been under process of finalization. Soon

after allotment of this contract there will be no need of Safaiwala at Central

Hospital, Jaipur. Although it has been informed by the CHI (CH)/JP that

these spare Safaiwala will be used for cleanliness of Circulating area. But

work study team has suggested that cleaning work of circulating are of the

Central Hospital premises should also be incorporated in the comprehensive

contract and 22 Hospital Safailwala have been considered surplus.

Similarly at Rewari Sub-divisional Railway Hospital against the sanctioned

strength of 04 Safaiwala, 03 are on roll and 01 Safaiwala is going to retire

in coming 02 year. Thus, it has been proposed to outsource the cleaning

work. 04 Medical Safailwala have been considered surplus.

This has been observed that day to day housekeeping of private/corporate

hospitals are remarkably much more better than the regular housekeeping

is being done by the departmental Hospital Safaiwala in government

hospital. Further to observe their work and maintain their service records

we need second line of staff as well. This increases the expenditure of the

organization remarkably.

At present 34 posts are sanctioned for Hospital Safaiwala over Central

Hospital and Jaipur division, out of which 27 staff are on roll and 07 posts

are lying vacant. Work study team is with a view that in the light of above

facts after outsourcing of the work, to cope up with remaining work 08

Hospital Safaiwala will be sufficient. Thus, 26 Hospital Safaiwala have been

considered surplus and advised to surrender forthwith.

cadre of Sanitary Safaiwala and 18 posts are going to retire in coming two

years. After critical analysis of work at all the units it is proposed that

partial cleaning work in Railway colonies of Jaipur, Bandikui, Rewari and

Phulera may be outsourced. 82 staff have been considered sufficient to

meet out with the remaining workload after outsourcing. Thus , 55 (37+18)

posts of Sanitary Safaiwala staff has been considered surplus from the

cadre and advised to surrender forthwith

CHAPTER 12 SUMMARY OF RECOMMENDATION

12.0 Summary of recommendations is as follows:-

Hospital Safaiwala: It has been informed by the coordinator

that cleanliness work of newly constructed building of Central

Hospital has been outsourced and similarly due to vacancy

and coming retirement at Rewari outsourcing of the work has

been recommended. Thus, according to work study out of 34

cadre 26 Hospital Safaiwalas considered sufficient to meet

out with the work load after outsourcing of cleanliness work

at Central Hospital, Jaipur and Suub-divisional Hospital,

Rewari. But, in the discussion with MD/JP he desired to hold

07 Hospital Safaiwalas at Central Hospital, Jaipur for

housekeeping of old building, circulating area, medical store

kitchen, Sewa Grah in old HQ buiding and to meet out with

emergency breakdown of outsourcing work. Similarly CMS

has also desired to hold 02 posts of Hospital Safaiwalas to

meet out any break down of outsourcing at Sub divisional

Hospital, Rewari. Work study team has considered their

suggestion and in clubbed in the recommendations thus cadre

of 17 (8+7+2) Hospital Safaiwalas has been proposed and 17

Hospital Safaiwalas have been considered surplus and

advised to surrender.

Civil defuse generated due to renewal work of quarter is

staked in the Railway colonies at FL and it is not being

lead to disposed off at proper place. This gives a shabby

look to the Railway colonies and effects the cleanliness

conditions as well. Therefore, it is to be ensured that all

civil defused released due to construction work should

not be staked in the colonies and immediately taken

away for disposal.

4. Recommendation No.-4:

Nallas and Sewage lines found open at FL & AWR

Railway colonies due to which dirt fall in and block the

flow of water. Hence, slabs must be put upon to cover

these Nallas and Sewage lines.

5. Recommendation No.-5:

It has been observed that a big area of land in Railway

colonies specially at FL & BKI are lying vacant and

causing huge cost for maintenance. It is being

recommended that these vacant lands may be proposed

for lease to arrange events like-marriage, tournaments

etc. so that these can be converted in to major earning

resources.
