In 1958, the AO ASIF (Association for the Study of InternalFixation) formulated four basic principles1, which have be-come the guidelines for internal fixation. They are:– Anatomical reduction– Stable internal fixation– Preservation of blood supply– Early, active pain-free mobilizationThe fundamental aims of fracture treatment in the limbs andfusion of the spine are the same. A specific goal in thespine is returning as much function as possible to the injuredneural elements.2

AO ASIF Principles as applied to the spine3

Anatomical alignmentIn the spine, this means reestablishing and maintaining thenatural curvature and the protective function of the spine. Byregaining this natural anatomy, the biomechanics of thespine can be improved, and a reduction of pain can be expe-rienced.

Stable internal fixationIn the spine, the goal of internal fixation is to maintain notonly the integrity of a mobile segment, but also to maintainthe balance and the physiologic three-dimensional formof the spine.3 A stable spinal segment allows bony fusion atthe junction of the lamina and pedicle.

Preservation of blood supplyThe proper atraumatic technique enables minimal retractionor disturbance of the nerve roots and dura, and maintainsthe stability of the facet joints. The ideal surgical techniqueand implant design minimize damage to anatomical struc-tures, i.e. facet capsules and soft tissue attachments remainintact, and create a physiological environment that facilitateshealing.

Early, pain-free mobilizationThe ability to restore normal spinal anatomy may permit theimmediate reduction of pain, resulting in a more active, func-tional patient. The reduction in pain and improved functioncan result when a stable spine is achieved.

Contraindications

The ARCH Laminoplasty System is not to be used: – For single- or two-level spondylosis without developmental

spinal canal stenosis

The ARCH Laminoplasty System is not to be used whenthere is:– Focal anterior compression– Established absolute kyphosis– Isolated radiculopathy– Loss of anterior column support resulting from tumor,

trauma, or infection

Indications

– Ossification of the posterior longitudinal ligament (OPLL)over multiple levels with maintained cervical lordosis

– Congenital canal stenosis with maintained cervical lordosis– Multilevel cervical spondylosis with maintained cervical

lordosis– Posterior compression from ligamentous hypertrophy with

maintained cervical lordosis

The ARCH Laminoplasty System is intended for use in thelower cervical and upper thoracic spine (C3–T3) after alaminotomy has been performed.

Laminotomy

1Surgical approach

The patient is positioned prone in head pins with the neckslightly flexed and posteriorly translated. The head of the bedshould be raised to provide a level aspect to the surgical site.

A standard midline approach should be used to expose thelaminae and the facets at the desired level. Care should betaken to preserve the facet capsules, soft tissue attachmentsto the facet joints, the spinous processes and the inter-spinous ligaments.

2Perform laminotomy

After adequate exposure, transect the lamina by creating asthin a cut as possible 1 cm lateral to the midline. Avoid contact with the underlying dura. On the contralateral side,decorticate the lamina by scoring, then cut a half-thicknesstrough, 1 cm lateral to the midline. Release the ligamentumflavum and bridging vessels, as required.

OptionTo facilitate performing a laminotomy a power tool might beused, e.g. Electric Pen Drive or Air Pen Drive with Burr Attachment and steel or diamond burr.

3Laminar expansion

Required instruments

388.170 Lamina Elevator

Place the tines of one side of the Lamina Elevator under theventral surface of the completely transected lamina withoutdisturbing the underlying dura. Place the opposite set oftines on the center of the contralateral, or hinged, lamina sothat it will not slip off during laminar expansion. Firmly graspthe lamina with the Lamina Elevator and expand the gap.

Note: Lack of laminar movement may indicate that deeperscoring is required at the hinge site.

With the lamina in expanded position the appropriate mini-plate size can be determined by inserting the Trial Implantsinto the laminar gap.

The size of the Trial Implant corresponds to the size of theminiplate.

2Select miniplate

Required instruments

Select a single or double bend miniplate by placing the plateson the laminar expansion using the Holder for Miniplate anddetermining the best anatomical fit.

Required instruments

Once the spacer and miniplate construct is complete, use theGraft Holder to place the construct at the site. Avoid disturb-ing the underlying dura. Remove the Graft Holder once theconstruct is securely held between the laminar edges.
