SCHEDULE OF RATES FOR SERVICE(Exclusive of Private Branch Exchange Equipment)

INSTALLATION CHARGES

A service connection charge covering a portion of the average cost of the initialexpense of establishing service for new subscribers and of fumisliing additional faciUtiesto old subscribers shall be as follows:

(a) For individual or exchange party line service $3.50(b) For each extension set 2.00fc) For each extension bell 1-50(d) For each loud ringing gong 2.50(c) To cover dlvoctory, accounting, circuit and switchboard expense in eases

where service is established by the use of instruments and lines alreadyin place in the subscriber's premises and no change is made in the t5T?e orlocation of such instruments....'. 1-00

(f) To reconnect service when discontinued for non-payment of account 1.00(g) When it is necessary to ei-cct poles or anchors in order to carry the wires

from tlte main telephone line on the road allowance or street to theai)p!icant's pi'emises, such ijolcs and anchors in connection therewith will besupplied and erected by the Department at the expense of the applicant.

Excess Mileage ExchangeThe excess mileage rate for Exchange Sendee shall he 50e per month for each

nuarter mile or fraction thereof.

(a) If you desire Stiition-lo-Stntion sewire, call "Long Distance"' in tlie regularway. Anyone at that number will do."

(b) If you desire to re:ich a particuhir person at a certain telephone number oraddress, call "Long Distujife" in tlic regular way. When tlie Long Distance operatoranswers, give the uuiubei' of the telephone from which you are speaking, also your nameami initials, together with the Initials and uame of tlie particular person desired and thecity or town where he can be located.

(c) If you desire to plaic an appointment or messenger call, or transmit a writtenmessage or telegram, call "Long Di.slance" in tbe regular way and give yonr order asheretofore.

Messenger Service—Upon reiiuest, and the prepayment of reasonable charges, amessenger will be sent when possible to notify a uou-subscriber to come to any I'uljlicI'ay Station.

\Miuu a particular-person is desired, the Long Distance operator must be so advisedwhen the call is placed, otherwise charges will be made from the time connection isestablished with the telephone desired, wiiether the party reqaired is obtained or not.
