THE REITMAN CENTRE CARERS GROUP: This eight-week program is for family caregivers of people living with earlyand mid-stage dementia. An accredited program of the Sinai Health SystemReitman Centre CARERS Program, this is a fully integrated, comprehensivedementia care program that introduces the innovative use of standardizedpatients - actors trained to simulate real-life situations, so that caregivers,guided by expert clinical coaches, will learn how to deal with commonchallenging situations. The focus is on practical skills based toolsand emotional supports needed to provide care for family memberswith dementia. CARERS (Coaching, Advocacy, Respite, Education,Relationship, Simulation) Program.

THE REITMAN CENTRE TEACH GROUP: The TEACH program is a four-week program for care partners ofpersons living with dementia. It is an innovative therapeuticgroup intervention aimed at providing education and enhancingcoping skills of care partners TEACH (Training, Education, andAssistance for Caregiving at Home) Program.

COUNSELLING AND CONSULTATION:Family and one–on-one dementia specific counselling is provided to care partners and persons living withdementia in both home and o�ice settings. We provide education about the disease, coping techniquesand therapeutic counselling for the care partners.

“THE FRIENDLY PLACE”-YOUNG ONSET ADULT DAY PROGRAM:A dynamic day program for adults diagnosed with young onset dementia(received diagnosis before the age of 65). This program runs two days per week(Tuesday and Thursday 9 a.m. to 4 p.m.) and o�ers participants a stimulating,comfortable environment with various activities and outings that areco-created by both program participants and sta�. PRIMARY CARE COLLABORATIVE MEMORY SERVICES:The PCCMS team is an inter-professional team who works collaboratively with specially trained primary care physicians in their memoryclinics to provide early diagnosis, treatment and support for daily challenges associated with memory concerns. The teams assistswith the most challenging aspects of dementia care while e�iciently integrating specialist and community resources,such as the Alzheimer Society’s First Link® program.

The goals of the Primary Care Collaborative Memory Clinics (PCCMC) are to support cognitive health and quality of life for adults and seniorsliving in the community while ensuring the patient’s family physician maintains a central role in their patient’s care.

Who should be referred for a PCCMC assessment?Adults not previously assessed by another memory clinic or service, who are experiencing symptoms of memory lossthat may or may not be a�ecting daily functions may be referred.

Referrals for PCCMC are accepted from: • Primary care providers • Local GAIN teams

POWERFUL TOOLS FOR CAREGIVERS:Central East LHIN Self-Management workshop. In the six weekly classes, caregivers develop a wealth of self-caretools to: reduce personal stress, change negative self-talk, communicate their needs to family members andhealth-care or service providers, communicate e�ectively in challenging situations, recognize themessages in their emotions, deal with di�icult feelings and make tough caregiving decisions.(o�ered annually)

PUBLIC AWARENESS:Alzheimer Society sta�, together with a team of trained volunteers, provide information about brain health, dementia awareness andAlzheimer Society services throughout Durham Region at community based information and wellness fairs; we also o�er freepresentations, on request, to interested community groups. Available in English and French.

DEMENTIA CARE WORKSHOPS FORHEALTH-CARE PROVIDERS:Fees may apply. Group rates are available.

To support an educated dementia workforce, the Alzheimer Societyeducation team will customize a presentation to meet the educationand training needs of health-care/social service students and care sta�.We are certified trainers for U-First! and Gentle Persuasive Approachesin Dementia Care (GPA). These workshops are o�ered at theASDR o�ice quarterly. Requests for organization-specificsessions are also accepted.

DEMENTIA FRIENDLY COMMUNITY:Organizations that wish to learn more about dementia friendlycustomer service are encouraged to host a one-hour DementiaAwareness session for their sta�. By better understanding dementiathey will be able to consider communication approaches and helpfulstrategies for creating a more welcoming experience for theircustomers who live with dementia. Ideal for faith communities,financial, health and service sector organizations.

CHOIR:This popular intergenerational program runs once annually for up to 10 weeks. Proven to be mutually stimulating and enriching for young at heart.Both participants from the Alzheimer Society and local high school students sing together in weekly practices, culminating in a concert finale.Singing songs from the past evokes pleasant memories, and performing in front of others boosts confidence and self-esteem.

HORTICULTURAL:Engaging in plant and nature-based activities enhances physical, emotional andcognitive health and well-being, especially for reducing stress and anxiety. We partner with popular WindReach, (a local working farm) which has aninclusive, welcoming and accessible environment for all. Specializing inhorticultural therapy for both persons living with dementia and carepartners, participants produce their own food then get to takeit home for consumption.

RESOURCE CENTRE:ASDR provides a wide range of print materials, books and DVDs (both fiction and non-fiction) about brain health,dementia and community resources, including books/booklets written by persons with dementia,care partners and subject experts in the field.

MINDS IN MOTION:This outstandingly popular two hour long program combines physical exercise followed by social recreation.This program is designed for persons living with dementia and care partners to attend together, so they both receivethe benefits. Seated accommodations o�ered for all fitness and recreational activities, as this program is gearedto benefit all involved. Small snack provided. BRAIN WAVE CAFÉS: One of the most important activities to promote brain health is to stay socially connected. These social and informalget-togethers usually occur out in the community, such as libraries or older adult centres. While the main focus isto provide participants with an opportunity to socialize and chat, games, activities or guest speakers are oftenplanned. This is a relaxing, accepting and fun outing designed for: persons living with dementia,care partners and family and friends. Light refreshments served.

CREATIVE MOVEMENT:Developed by Baycrest and Canada’s National Ballet School (NBS),the Creative Movement program is a safe and accessibledance program that engages older adults in meaningfuldance activity that benefits physical, cognitive andoverall health. Programming is delivered throughvideo stream. Also provides a creative and artistic outletthat is fun and builds a sense of community.

IG WEALTH MANAGEMENT WALK FOR ALZHEIMER’S:The IG Wealth Management Walk for Alzheimer's is Canada’s biggest fundraiser forAlzheimer’s disease and other dementias. Monies raised support programs and servicesthat improve quality of life for people living with dementia and their families, as well assupport other activities like education and public awareness in Durham Region.Over 320 Walks take place in communities across Canada each year.
