For example, studies indicate that seven illness episodes are treated at home for each one that requires professional attention. For this reason, the Nagel Clinic believes focusing on policy and practice to support families in this pivotal role provides a powerful and cost-effective means of preventive health care. One such family that benefits from this approach to healthy living is that of Juan and Paola Gutierrez, along with their three children: Allie (age 11), Juanito (age 5), and (the newest member of the family) little Salomon. While the Clinic does not offer pediatrics, our care of Paola with her pregnancy test and pre-natal advice contributed to the good health of both mother and child. Juan is on the staff of Helping Hand of Bandera County, while Paola is a longtime volunteer. We recently spoke with Juan about the importance of the Nagel Clinic to his family. When did you and Paola find out about the Clinic? We started around five years ago, when I began working at Helping Hand. He and his wife, JoAnn, reside in Pipe Creek and are real estate developers in the Hill Country area. Also newly elected to the Board is Michael R. Mulherin, a retired executive with Dow Chemical Company’s cogeneration plants, energy technology center, and energy business. Mr. Mulherin brings a wealth of knowledge in budgets, audits, assessments, and senior administrative review for a corporate giant to the Clinic’s Board. He and his wife, Rachel, moved from the Gulf Coast to Lakehills full-time, where they built a home in 2012. They are active with the Lake Medina Conservation Society, Medina River Protection Fund, and Bandera County River and Ground Water District. Additionally, they attend St. Christopher’s Episcopal Church, where Mike serves on the Vestry and Outreach Committee. recently spoke with Juan about the importance of the Nagel

When did you and Paola find out about the Clinic?

We started around five years ago, when I began working

at Helping Hand. I don’t come very often but I was recently

The Nagel Clinic’s medical and administrative staff, with

key volunteers, gathered for an all-day strategic planning

session on March 11 at the gorgeous Lodge on San Julian

Creek. This critical planning, involving the staff, volunteers,

and Board of Directors, occurs every 4-5 years and deter-

mines the future course of the organization. Our time at

the Lodge was generously underwritten by the Albert &

Margaret Alkek Foundation.

Strategic Planning Success!

The Arthur Nagel Community Clinic has announced the

reopening of their Nagel Clinic at Lakehills, which has been

shuttered since March of last year due to the COVID-19

pandemic and the temporary closing of the Lakehills

Community Center. Located in what is arguably the most

populous area of Bandera County, the branch clinic is again

operating inside the Lakehills Community Center on the

first and third Mondays of each month from 11:30 am until

3:30 pm. The Lakehills Community Center is located at

11225 PR 37, in Lakehills near the shores of Medina Lake.

With the rate of COVID-19 vaccinations increasing each

week, our friends on the Board of Directors of the Lakehills

Community Center have decided to reopen to the public and

we are so pleased to be offering no-cost health care services

to families in Pipe Creek, Bandera Falls, and Lakehills who

can again receive care much closer to where they live. We

urge existing patients and others in the area needing care to

call the main Clinic in Bandera at (830) 796-3448 to set up an

appointment for the Nagel Clinic at Lakehills.

To be eligible to receive services, all Nagel Clinic

patients must provide documented proof of identity,

residency in Bandera County, and family income at or

below 250% of the federal poverty level. Since the

founding of the Nagel Clinic in 2008, medical services have

been provided without charge thanks to generous funding

Lakehills Community Center Board President, Kelly Beedle,

shared her excitement about the Clinic’s return to the Lakehills

From The Executive DirectorThese are exceptional days for the Nagel

Clinic. Thank you for partnering with us as

we weathered the challenging year of 2020,

strengthened our financial position, and

developed a new strategic plan to take us

through the next four years of meeting the

health care needs of the most vulnerable in

our community.

We all want life to get back to normal. The good news is we’re getting there. Millions of Americans

have received the COVID-19 vaccines. However, more people still need to be vaccinated in order

to stop the spread of the virus. I’m proud to say that the Nagel Clinic has joined forces with other

organizations across the region to increase awareness and improve vaccination rates.

As the Nagel Clinic continues to redefine its community fundraising event to popular acclaim and

sold-out houses, we are pleased to announce Cork & Fork is back for 2021. Under the theme of

“Will The Circle Be Unbroken: Family, Faith, Freedom”, we will return to The Farm Country Club

on Saturday, November 13. Watch for ticket information in the months ahead. The Clinic is bringing

back music and fun for a worthy cause, and we cannot wait for you to join us.

Lastly, we are delighted that the Carl & Marie Jo Anderson Foundation has, in a new partnership

with us, issued a $20,000 challenge grant for our mission. If the Clinic raises $20,000 from first-

time donors, lapsed donors, or increased gifts over last year, we will receive this needed grant to

A team of five graduating seniors from the University of Texas at Austin’s

McCombs School of Business MIS program has generously assisted

the Nagel Clinic by developing and implementing a professional grants

management system within the Clinic’s existing Salesforce donor

database. In 2021, more than 78% of the Clinic’s contributed revenue will

come from foundations and corporations.

Space for the Nagel Clinic’s services at Lakehills is generously

provided by the Community Center. A new medical exam bed

was donated to the Nagel Clinic for the Lakehills location in 2019

by St. Christopher’s Episcopal Church in Bandera.

Tuttle, the MIS class project took three months. It involved an in-depth

examination by the students of the multiple steps in the nonprofit

grants process. These include comprehensive funding research,

initial outreach, cultivation, completing the detailed application with

requested budgets and other attachments, grant award, reporting

on the use of funds, recognition, and ongoing relationship with the

foundation or corporation.

The Nagel Clinic is deeply indebted to Professor Tuttle and the

McCombs School of Business at UT Austin for sharing the energy,

enthusiasm, knowledge, and skill of these outstanding students for this

invaluable project to advance our mission.

Why the Nagel Clinic?

Twenty percent of Texas residents are uninsured – twice the

national average, and the highest state percentage in the

country. According to the Texas Association of Community

Health Centers, about 1.3 million low-income Texans – 42%

of whom are uninsured – rely on community health centers

as their primary health care resource.

Nearly 15% of those living in Bandera County, or

roughly 3,466 individuals based on July 2019 census data,

have incomes at or below 250% of the poverty level,

making them eligible for the Nagel Clinic’s services and

programs. The most recent statistics estimate Bandera

County’s unemployment rate to be 6.0% (Texas Workforce

Commission 1/2021), slightly lower than the overall jobless

rate of 6.6 % for the 13-county Alamo metro area (Alamo

Workforce Solutions, 1/2021).

The patient-to-physician ratio for the county is 3,500:1;

mental health providers at 2,000:1; and the patient-to-dentist

ratio is 7,000:1 (Robert Wood Johnson Foundation, County

Health Rankings 2018.) The Texas Education Agency reports

that 51% of students in Bandera and Medina Independent

School Districts are economically disadvantaged, and 43%

are at risk of not finishing high school. Based on U.S. Census

figures, the county federal poverty rate is nearly 15% of the

23,000 residents.

For Bandera County residents, the closest area hospital

is either a two-hour roundtrip to San Antonio, or a one-hour

roundtrip to Kerrville. This presents a hardship for those

without reliable transportation, sufficient financial means,

and/or schedules that permit lengthy doctor visits.

Additionally, the cost for a trip to the emergency room is

on average 12 times higher than being treated at a medical

provider’s office for common ailments (Houston Chronicle).

For the uninsured, the costs increase more since there is

no carrier to negotiate lower prices. Hospital losses are

frequently offset by federal aid coming from taxpayers.

By continuing to provide health care services the Nagel

Clinic ensures that long-term adverse health consequences,

and attendant financial consequences, are minimized for

the most vulnerable individuals and families in rural Bandera

County.

Dr. Michael A. McConnell, Col. (retired), has joined the staff

of the Nagel Clinic as a distinguished volunteer provider for

dermatology. His wife, Carol, a Registered Nurse, assists him

with examining and treating Clinic patients at visits twice a

month. We are so fortunate to have their combined medical

expertise and good humor!

Dr. Mike brings with him decades of service to America’s military

hospitals and personnel, including assignments in San Antonio,

New York, San Francisco, Panama, Germany, and Saudi Arabia,

among many others. He studied medicine at Trinity College

in Dublin, Ireland, with a Walter Reed Army Medical Center

residency in Washington D.C. He is Board Certified in both

Dermatology and Pathology. Among his many military awards

of merit are the Silver Star, Bronze Star, Combat Medical Badge,

Parachute Wings, and Flight Surgeon Wings.

After successfully reimagining our largest annual fundraising

event in 2018 and 2019, the Nagel Clinic canceled the 2020

Cork & Fork because of the global pandemic. Now, with

the mitigation of the virus through safety measures and

vaccinations, the Clinic very much looks forward to welcoming

the community once again to The Farm Country Club this fall for

another memorable and record-breaking Cork & Fork.

“We’re back!” is the collective declaration of the Clinic’s Board

of Directors, Volunteers, and Staff in anticipation of this year’s

showcase event to be held on Saturday, November 13. When

it became necessary to cancel last year’s event, many loyal and

generous supporters stepped up to replace the funds lost to

the operating budget by the cancellation, and we met our goal!

In appreciation of that support, invitations to the 7th annual Cork

& Fork will go first to those who gave to the Rally For The Clinic

Campaign last fall, then to sponsors and ticket buyers for Cork

& Fork in 2018 and 2019. Tickets will go on sale to the public

after Labor Day weekend.

This year’s theme is “Will The Circle Be Unbroken: Family,

Faith, and Freedom” with a reflection on what makes the

Bandera and Hill Country communities so uniquely strong and

united in spirit. New beginnings are cause for celebration, and

we cannot wait to share the joyful return of this remarkable

event with you.

The past 16 months of living with COVID-19

has brought many changes for everyone.

We saw significant changes in mental health

in Bandera County, similar to trends seen

around the world. We made several more referrals of

Clinic patients to the Texas Department of Mental Health

& Developmental Disability for further evaluation than we

had in previous years – and quite possibly more than the

prior two years combined.

During the beginning of the pandemic, our weekly

counseling sessions decreased. This was to be expected,

when you look closely at human development. There is

a theory in psychology known as Maslow’s Hierarchy. At

the core level of the theory, humans need to have their

basic needs met. They need food, shelter, clothing,

and safety. At this time, people began to focus on

their immediate safety from the virus, and whether

there would be a disruption to the supply chain.

They isolated at home as requested.

Soon summer arrived. As folks began to realize

they were relatively safe and needs were being met, a

new trauma response surfaced for many. After months of

being locked down and isolated, people found they had

developed acute depression. The only connection some

had to others and the outside world was through social

media and the news networks. Suddenly, we saw a surge

in appointments for depression and anxiety at the Clinic.

This was exactly on trend. Our weekly sessions doubled

as more and more of our patients sought help with the

newly formed depression and anxiety.

In the fall, our weekly sessions began decreasing

and returning to levels seen in previous years. Also, we

had moved to telehealth. This had an added bonus for

our patients that we did not anticipate: approximately half

of our patients have asked to remain on telehealth. They

reported that telehealth has been easier for them with

their busy work schedules and families. It has also been

easier for them financially as they do not have to pay for

rides or gas to drive to the Clinic.

Now we have watched as the vaccine rollout has

intensified during the spring of 2021. Case numbers and

deaths have decreased substantially in the U.S. More of

our pre-COVID life and activities are resuming. Interactions

with family and friends are increasing. While we will never

be free of stressors that affect daily living, at least for now

they are compounded less by fear of a debilitating and

sometimes fatal disease.

The Nagel Clinic has been able to upgrade

technology and add value to the patient

experience with the acquisition of needed

equipment.

To save time and expense for results from an

outside lab, an A1C Analyzer was purchased for

our in-house lab.

Likewise, patients no longer must travel to

and from Boerne for some imaging as a new

portable Ultrasound has been added. This device

is especially helpful for our patients in the Well-

Woman Program.

Lloyd and Sheri Norris have gifted the Clinic with

a new, digital 1,000-lb capacity weight scale with

needed handrails for our infirm patients.

And finally, thanks to Methodist Healthcare

Ministries, the Clinic replaced its timeworn ECG

with a brand new model.

Each year, the Nagel Clinic is pleased to welcome nursing

students from area universities for training and observation

as part of their clinical rotation hours toward their degrees.

Linda Smith is just what the doctor ordered: an eager, friendly,

and skilled volunteer for the Nagel Clinic who comes with

a Master’s Degree in Health Care Administration from the

University of California at San Bernardino. She hails from

Apple Valley, California, which is famous for a couple of other

residents: Roy Rogers and Dale Evans. Linda feels right at

home in Bandera, the Cowboy Capital of the World.

We are so fortunate to have Linda, who knows her way

around complex electronic medical records and healthcare

terminology, plus is an always smiling face when she interacts

with patients at the reception desk or on the phone.

Linda is a relatively new resident of Texas, who discovered the

Clinic when she came to donate some medical supplies after

the death of her husband. Her remarkable talents became so

needed that she quickly went from one day to two and a half

days per week. She cites her love for the Nagel Clinic in this

order: 1) the people she gets to work with; 2) the fulfillment of

the work itself; and 3) the patients are so nice!
