SOSSOS
Occurs most commonly as a complication of myeloablative regimens that are used to y gprepare patients for bone marrow transplantationtransplantation (highdose chemotherapy drugs or chemotherapy drugs plus total body irradiation)
Other causes of SOSOther causes of SOS

the sinusoid precedes liver dysfunction
Clinical PresentationClinical Presentation

Sinusoidal Obstruction Syndrome presents with a
wide spectrum of severity
Mild SOS is considered disease that:
• meets diagnostic criteria, • does not require treatment for fluid excess or medication for hepatic pain, p p ,
• has a self‐limiting course.
Sinusoidal Obstruction Syndrome presents with a
wide spectrum of severity
Moderate SOS is disease with:• evidence of liver injury, evidence of liver injury,• requires treatment for fluid excess or
di i f h i imedication for hepatic pain, • resolves completely. p y
Sinusoidal Obstruction Syndrome presents with a
wide spectrum of severity
Severe SOS is defined as:• disease that leads to deathdisease that leads to death • or that does not resolve by day 100
Sinusoidal Obstruction Syndrome is a clinical diagnosisSinusoidal Obstruction Syndrome is a clinical diagnosis
with all the accompanying uncertainty and need f li i l j dfor clinical judgment.
Clinical Criteria for Diagnosis of Sinusoidal Obstruction Syndrome
l i i il i i Baltimore CriteriaSeattle Criteria
● Hyperbilirubinemia >2 mg/dL plus >= 2 other criteria:
Two of three findings within 20 days of transplantation:
● Hepatomegaly, usually painful ● >=5% weight gain
● Bilirubin >2 mg/dL● Hepatomegaly or RUQ pain of liver
● Ascitesorigin ● >2% weight gain due to fluid accumulation
Best‐studied modality is gray‐scale and color Doppler US.
The diagnosis may be supported by imaging, but imaging is currently not diagnostic by g g y g yitself.
The majority of studies suggest that no single ultrasound parameter is diagnostic for Sinusoidal Obstruction Syndrome.diagnostic for Sinusoidal Obstruction Syndrome.
When the diagnosis is uncertain in a moderately or severely ill patient
• Evaluation by transvenous liver biopsy with hepatic venous pressure gradient p p gmeasurement.
• Percutaneous liver biopsy is contraindicated in presence thrombocytopenia, coagulopathy or ascites.ascites.
Oxaliplatin
Despite the increasingly extensive use of chemotherapy in colorectal cancer, its py
functional and histopathological effects on the liver remain inadequately describedliver remain inadequately described.
For patients with resected node‐positive colon cancer data from the randomized MOSAIC and NSABP C‐07 trials indicate the superiority of adding oxaliplatin to a 5‐FU plus leucovorinadding oxaliplatin to a 5 FU plus leucovorin
backbone.
• Whether oxaliplatin is beneficial for adjuvant treatment of rectal cancer is not yet y
established.
Despite the lack of data, guidelines from the NCCN include FOLFOX (oxaliplatin plus short‐( p pterm infusional 5‐FU and leucovorin) as an acceptable regimen for the chemotherapyacceptable regimen for the chemotherapy component of adjuvant therapy in rectal
cancercancer.
During clinical trials, the adverse events most often reported were hematological toxicity, p g y
gastrointestinal tract toxicity and a neuropathy none of which has been observedneuropathy, none of which has been observed with other therapeutic platinum derivatives
At least three‐quarters of patients who received oxaliplatin have hepatic sinusoidal p p
abnormalities (termed sinusoidal obstruction syndrome) similar to those that characterizesyndrome) similar to those that characterize
veno‐occlusive disease.
The majority of instances of sinusoidal dilatation, vascular injury and congestion j y g
found histologically after oxaliplatin therapy occur without significant serum enzymeoccur without significant serum enzyme
elevations or clinically apparent liver injury.
Rare instances of acute onset of sinusoidal obstruction syndrome with ascites and hepatic y pfailure have been described after oxaliplatin therapy usually when given in combinationtherapy, usually when given in combination
with other antineoplastic agents.
Sinusoidal injury complicated by fibrosis and veno‐occlusive lesions should be included in the list of the adverse side‐effects of systemic
chemotherapy for colorectal tumorschemotherapy for colorectal tumors, particularly in connection with the use of
oxaliplatinoxaliplatin.
RecommendationsRecommendations
• Consider the diagnosis of SOS in patients in the appropriate clinical scenario who present pp p pwith tender hepatomegaly, fluid retention and weight gain and elevated serum bilirubinweight gain, and elevated serum bilirubin
RecommendationsRecommendations
• Rule out other common causes of jaundice (such as biliary tract obstruction, hemolysis, ( y ysepsis, drug induced liver injury and viral and fungal infections involving the liver) andfungal infections involving the liver) and weight gain (for example due to fluid overload renal insufficiency or congestiveoverload, renal insufficiency or congestive heart failure) in this population
RecommendationsRecommendations
• Image the liver with Doppler ultrasound or another modality to rule out other causes and yto demonstrate features consistent with SOS
RecommendationsRecommendations
• In complicated cases, perform a transvenous liver biopsy with hepatic venous pressure p y p pgradient to confirm the diagnosis (hepatic venous pressure gradient of greater than 10venous pressure gradient of greater than 10 mm Hg is highly specific for SOS
RecommendationsRecommendations
• Consider regimens that are less likely to cause toxic liver injury in patients with extensive j y phepatic fibrosis, viral hepatitis or a previous history of SOShistory of SOS
RecommendationsRecommendations
• Transjugular intrahepatic portosystemic stent shunts (TIPS) or tissue plasminogen activator ( ) p gare not recommended for the therapy of SOS (in SOS damage to hepatocytes is caused by(in SOS damage to hepatocytes is caused by ischemia due to obstruction at the sinusoidal level )level )
