Locating: This includes assisting the individualand his or her representative with connecting to,arranging for, and obtaining services specified in theISP, including needed medical, social, habilitation,education, or other community services.

The SC will need to assist the individual in identifying and inviting people to be a part of his or her ISP team who can be valuable supports

• These can be family members, friends, professional contacts, etc.

• REMEMBER: The individual is the lead of his or her team. Be sure to honor the individual’s wishes.

Once the individual chooses his or her needs-based services, the SC will need to assist in locating these…

• Using the Supports and Services Directory, the SC will locate the contact for each provider

• The SC can call or email contact to ask if they have availability to accept a new individual and inform the individual of all available providers

Once the individual is linked to their providers, the SC will need to locate the contingency for each service

• The “contingency” is the person who can be contacted if the main staff is not available.

• This person will be added in the service details screen.

The SC is responsible for locating services of both AAW services as well as non-waiver services

• The ISP should include information about discussion of need for non-waiver services, as well as use of non-waiver services

• The SC is responsible for knowing county and state funding resources such as County MH services and Medicaid funding
